          Abstract

          The authors report a case of Shy-Drager syndrome in a 53 year-old male patient. Autonomic failure was made evident by physical examiration as well as laboratory tests. A sleep recording showed decreased percentage of REM sleep and apneas of the central type. The possible mechanisms for this sleep disorder are discussed.

          É relatado o caso de um paciente de 53 anos de idade com diagnóstico de síndrome de Shy-Drager. Testes laboratoriais e exame físico evidenciaram claramente o distúrbio do sistema nervoso autônomo. Foi também obtido traçado poligráfico do sono durante uma noite completa, que mostrou diminuição do sono tipo MOR e apnéias do tipo central. Os possíveis mecanismos destes achados polissonográficos são discutidos.

          Orthostatic hypotension from a cerebellar gangliocytoma (Lhermitte-Duclos disease). Case report.

          A 49-year-old woman presented with orthostatic hypotension. Vertebral angiography and ventriculography revealed a tumor of the left cerebellar hemisphere. Ventriculoperitoneal shunting was followed by complete disappearance of the orthostatic hypotension. The tumor was subsequently removed and microscopic study showed Lhermitte-Duclos disease. Orthostatic hypotension has been rarely reported in association with tumors of the posterior fossa except for those tumors destroying the medullary centers and interrupting the baroreceptor reflex arc. This case is of interest because the tumor was restricted to the cerebellum. The authors have found no previous case in which orthostatic hypotension was a presenting symptom of Lhermitte-Duclos disease.

              Postural hypotension and low R-R interval variability in parkinsonism, spino-cerebellar degeneration, and Shy-Drager syndrome.

              Systolic blood pressure, mean R-R interval, and R-R interval variance were studied in patients with Parkinson's disease, spinocerebellar degeneration, and Shy-Drager syndrome. Postural hypotension correlated with anhidrosis (p less than 0.05), indicating sympathetic vasomotor dysfunction. Reduction of R-R interval variance while resting supine correlated with bladder-bowel dysfunction (p less than 0.05), indicating parasympathetic impairment. Reduction of R-R interval variance after postural changes correlated with constipation (p less than 0.005), postural hypotension (p less than 0.05), and anhidrosis (p less than 0.05), indicating both parasympathetic and sympathetic involvement. Dynamic study of the R-R interval provides objective information about autonomic function in neurologic disease.
