Abstract

Introducción. Aunque el cáncer de testículo (CT) no es un cáncer frecuente en España y además tiene buen pronóstico, la posibilidad de realizar un diagnóstico precoz le podría hacer susceptible de adopción de medidas para adelantar su diagnóstico: autoexamen testicular o screening mediante ecografía testicular.Objetivos. Revisar la evidencia científica disponible y explorar qué método es el más efectivo para la detección precoz del CT.Metodología. Revisión sistemática de bases de datos en castellano e inglés, mediante los portales de salud ADITUS y FISTERRA, desde 1993 a 2003. Se definió una estrategia de búsqueda para cada base de datos. La valoración de la calidad de los artículos se midió con la tabla de jerarquía de la evidencia de Cooke y Sackett. Se realizó un resumen de los datos utilizando la tabla de Beck, Hgler y Polit. La descripción de los estudios se valoró con la tabla de Crombie. Resultados. Se incluyeron 7 estudios, 5 escritos en inglés, uno en español y otro en ambos idiomas. El mayor número de artículos publicados se encontró en las bases de datos inglesas. Cinco artículos alcanzaron el grado B de evidencia y dos el D. En general, la descripción de los artículos fue adecuada y clara. Conclusiones. No existe evidencia científica suficiente que permita conocer cuál es el mejor método de detección precoz del cáncer de testículo, aunque se sugiere que el autoexamen testicular y el screening mediante ecografía testicular son métodos efectivos para una detección precoz de este cáncer.

Background. Testicular cancer is the most common malignancy in men between the ages of 15 and 35, yet it is one of the more curable cancers. There are two methods to detect testicular cancer, testicular self-examination and screening by an ultra sound scan of the testes. Objective. To review the available evidence to explore what method is the best effective for an early detection of testicular cancer.Methodology. Databases from 1993 till 2003 were searched in English and Spanish language, through the ADITUS and FISTERRA Health Knowledge Portals. A search strategy was defined for every database. Quality assessment of the articles was measured using the hierarchy of evidence table of Cooke and Sackett. A data abstraction was obtained using the table of Beck, Hgler and Polit. A description of the studies was assessed with the table of Crombie. Findings. Seven studies were included, 5 were written in English, one in Spanish and other one in both languages. Almost all of the published articles was found in the english databases. Five articles reached the B grade and two the D grade of evidence. In general, the assessment of the selected articles was adequate and clear. Conclusions. There is no available evidence to know which method is the best effective for an early detection of testicular cancer. However, testicular self-examination and screening are effective methods for an early detection of this cancer.
