          Abstract

          OBJETIVOS: Verificar a prevalência de sintomas musculoesqueléticos entre trabalhadores de enfermagem de um hospital universitário público do interior do Rio Grande do Sul, e identificar variáveis sociodemográficas e laborais associadas a esses sintomas. MÉTODOS: Estudo transversal, envolvendo 491 trabalhadores de enfermagem de um hospital universitário do Rio Grande do Sul. Utilizou-se a versão brasileira do Nordic Musculoskeletal Questionnaire para identificação dos sintomas musculoesqueléticos. RESULTADOS: Entre os participantes, 96,3% referiram sentir dor em alguma região do corpo no último ano, 73,1 % nos últimos sete dias e 65,8% relataram dificuldade nas atividades diárias. A coluna lombar foi a localização mais freqüente referida pelos trabalhadores. Características sociodemográficas (ser mulher, extremos de idade, filhos pequenos, baixa escolaridade, obesidade, tabagismo) e laborais (ser técnico ou auxiliar de enfermagem, trabalho noturno, alta demanda física no trabalho) estiveram associadas a dor em várias regiões. CONCLUSÃO: Os resultados indicam necessidade de propostas participativas para a promoção da saúde e bem-estar no trabalho de enfermagem, envolvendo tanto gerentes hospitalares quanto trabalhadores.

          OBJECTIVES: To verify the prevalence of musculoskeletal symptoms among nursing workers at a teaching university hospital in Rio Grande do Sul, and to identify socio-demographic and labor variables associated with those symptoms. METHODS: Transversal study involving 491 nursing workers at a teaching university hospital in Rio Grande do Sul. It was used the Brazilian version of the Nordic Musculoskeletal Questionnaire to identify the musculoskeletal. RESULTS: Among participants, 96.3% reported pain in some part of the body in the last year, 73.1 % in the last seven days and 65.8% difficulties in daily activities; pain in the spinal column was the most frequent mentioned by workers; socio-demographic characteristics (being a woman, extremes of age, minor children, little education, obesity, tabaccoism) and labor characteristics (technician or auxiliary nurse, night shift work, strenuous physical labor) were associated with pain in various regions. CONCLUSION: The results indicated for participatory proposals for promoting health and well-being in nursing work environment; hospital managers and workers should participate.

          OBJETIVOS: Verificar la prevalencia de síntomas músculo-esqueléticos en trabajadores de enfermería de un hospital universitario público del interior de Rio Grande do Sul, e identificar variables sociodemográficas y laborales asociadas a esos síntomas. MÉTODOS: Estudio transversal, envolviendo 491 trabajadores de enfermería de un hospital universitario de Rio Grande do Sul. Se utilizó la versión brasileña del Nordic Musculoskeletal Questionnaire para identificar los síntomas musculo esqueléticos. RESULTADOS: Entre los participantes, 96,3% refirieron sentir dolor en alguna región del cuerpo en el último año, 73,1 % en los últimos siete días y 65,8% relataron dificultad en las actividades diarias. La columna lumbar fue la localización más frecuentemente referida por los trabajadores. Las características sociodemográficas (mujer, extremos de edad, hijos pequeños, baja escolaridad, obesidad y tabaquismo) y laborales (técnico o auxiliar de enfermería, trabajo nocturno y alto esfuerzo físico en el trabajo) estuvieron asociados al dolor en varias regiones. CONCLUSIÓN: Los resultados indican la necesidad de presentar propuestas participativas, por parte de administradores hospitalarios y trabajadores, para la promoción de la salud y aumentar el bienestar en el trabajo de enfermería.

          Cross-cultural adaptation of the Nordic musculoskeletal questionnaire.

          Reports in the literature have identified a need for internationally standardized and reliable measurements to analyse musculoskeletal symptoms. Screening of musculoskeletal disorders may serve as a diagnostic tool to evaluate the work environment. The Nordic general questionnaire is a standardized instrument used to analyse musculoskeletal symptoms in an ergonomic or occupational health context. To translate and adapt a version of the Nordic general questionnaire into Brazilian Portuguese and evaluate its reliability. The cross-cultural adaptation was performed according to internationally recommended methodology, using the following guidelines: translation; back-translation; committee review; and pretesting. First, the questionnaire was independently translated into Portuguese by two teachers and one doctor, and a consensus version was generated. Second, two other translators performed a back-translation independently from one another. This version was then submitted to a committee, consisting of six specialists in the area of knowledge of the instrument, to evaluate its equivalence to the original instrument. The final version was pretested on 20 subjects randomly selected in an outpatient clinic. Reliability was assessed by a test-retest procedure at 1-day intervals using the Kappa coefficient in a group of 40 subjects. The Kappa agreement values were calculated for each one of the four questions of the questionnaire. The agreement among the same observers was substantial, varying from 0.88 to 1, according to the Kappa values. these demonstrated strong agreement of the instrument, suggesting that the Brazilian version of the "Standardized Nordic Questionnaire" offers substantial reliability.

            Psychosocial stressors at work and musculoskeletal problems.

            This paper examines the relationship between work stressors and the following health indicators: psychosomatic complaints, health behavior, and musculoskeletal problems. Secondary analyses were performed on data from the National Work and Living Condition Survey, which provides a representative sample of the working population in The Netherlands. The survey was made in 1977, 1983, and 1986 by The Netherlands Central Bureau of Statistics. By means of factor analysis the following three risk dimensions were identified in the survey: work pace, intellectual discretion, and physical stressors. High work pace, low intellectual discretion, and physical stressors were associated with increased health complaints (both psychosomatic and musculoskeletal) and musculoskeletal disorders after adjustment for gender, age, education, and sports participation. Low intellectual discretion, but not high work pace, was associated with poor general health and health behavior indicative of poor health. Physical stressors were associated with general health as well, but not with health behavior, except for reported absenteeism. Psychosocial stressors are not only associated with psychosomatic complaints and health indicators, but also with musculoskeletal problems, both acute and chronic. Especially the relation between intellectual discretion and musculoskeletal problems can be partly attributed to physical load. Even after adjustment for physical stressors and moderating personal characteristics, the relationships between the psychosocial stressors and musculoskeletal problems remained significant and comparable in strength to the relationship between psychosocial stressors and several other health outcomes, such as psychosomatic complaints.

              Prevalência de sintomas músculo-esqueléticos em trabalhadoras de enfermagem

              Este trabalho teve por objetivo avaliar sintomas músculo-esqueléticos em trabalhadoras de enfermagem. Os sujeitos compreenderam 105 auxiliares e técnicos de enfermagem que trabalhavam em unidades de internação de um hospital que atende pacientes com alto grau de dependência física. Utilizou-se um questionário contendo dados demográficos e ocupacionais e queixas osteomusculares, dentro de uma abordagem ergonômica, adaptado do Nordic Musculoskeletal Questionnaire. Das participantes, 93% referiram algum tipo de sintoma osteomuscular nos últimos 12 meses. As mais elevadas prevalências desses sintomas, segundo as áreas anatômicas, foram: região lombar (59%), ombros (40%), joelhos (33,3%) e região cervical (28,6%). Verificou-se também que, das respondentes, 29,5% faltaram ao trabalho e 47,6% consultaram um médico nos últimos 12 meses devido aos mesmos sintomas. Observou-se também menor tempo de trabalho na área de enfermagem e na unidade atual entre as trabalhadoras com ocorrência maior de dor nos joelhos (p=0,0272) e dor lombar (p=0,0332), respectivamente. No entanto, a dor em punhos/mãos mostrou-se mais freqüente entre as participantes com maior número de horas semanais trabalhadas (OR=3,72
