          Abstract

          This research evaluated the combined effects of instructions (through health education manual) and self-monitoring (using record forms) in occurrence of adherence to treatment by adolescents diagnosed with Juvenile Systemic Lupus Erythematosus. Four adolescents participated in the research, which had four stages: 1. Initial assessment; 2. Intervention, including Condition 1, with exposure to the manual followed by the self-monitoring record forms, and Condition 2, with a reversal order of these instruments; 3. Feedback; and 4. Follow-up. The data obtained with 24h recall and self-monitoring record forms were quantified, allowing the calculation of the Treatment Adherence Index. Three participants showed an increase in this index after the use of self-monitoring records, regardless the exposition order of the instruments. The results of this research suggest that self-monitoring can assist in the installation of adherence behaviors, while manuals with instructions can help to maintain those behaviors, which would justify the combined use of these instruments.

          Esta investigación tuvo por objetivo evaluar los efectos del uso combinado de instrucciones (vía manual de educación en salud) y de automonitorización (vía formularios de registro) sobre la ocurrencia de relatos de adhesión al tratamiento por adolescentes con diagnóstico de Lupus Juvenil. Cuatro adolescentes participaron de la investigación que se realizó en cuatro etapas: 1. evaluación inicial; 2. Intervención, incluyendo - Condición 1, con exposición al Manual seguido de los Formularios de Registro de Automonitorización, y Condición 2, con inversión de la aplicación de los instrumentos; 3. Feedback; 4. Follow-up. Los datos obtenidos a partir del recordatorio de 24h y de registros en los Formularios de Automonitorización fueron cuantificados, permitiendo el cálculo del Índice de Adhesión al Tratamiento. Tres participantes mejoraron la adhesión después de la automonitorización, independientemente del orden de presentación de los instrumentos. Los resultados sugieren que la automonitorización puede auxiliar en la instalación de comportamientos de adhesión, mientras que el manual con instrucciones puede auxiliar en el mantenimiento de los mismos, lo que justificaría el uso combinado de estos instrumentos.

          Esta pesquisa avaliou os efeitos do uso combinado de instruções (via manual de educação em saúde) e de automonitorização (via formulários de registro) sobre a ocorrência de relatos de adesão ao tratamento por adolescentes com diagnóstico de Lúpus Juvenil. Quatro adolescentes participaram da pesquisa realizada em quatro etapas: 1. Avaliação inicial; 2. Intervenção, incluindo Condição 1, com exposição ao Manual seguido dos Formulários de Registro de Automonitorização, e Condição 2, com inversão da aplicação dos instrumentos; 3. Feedback; e 4. Follow-up. Os dados obtidos por meio de Recordatórios 24h e de registros em Formulários de Automonitorização foram quantificados, permitindo o cálculo do Índice de Adesão ao Tratamento. Três participantes melhoraram a adesão após a automonitorização, independentemente da ordem de apresentação dos instrumentos. Os resultados sugerem que a automonitorização pode auxiliar na instalação de comportamentos de adesão, enquanto o manual com instruções pode auxiliar na manutenção dos mesmos, o que justificaria o uso combinado destes instrumentos.

          Measuring disease activity in adults with systemic lupus erythematosus: the challenges of administrative burden and responsiveness to patient concerns in clinical research

          Measuring lupus disease activity accurately remains a challenging and demanding task given the complex multi-system nature of lupus, an illness known for its variability between patients and within the same patient over time. Many have attempted to define what disease activity means and how it should be measured, and several instruments were devised for a standardized assessment of disease activity and outcome domains in clinical research. Several of these measuring tools have been able to detect clinical improvement and have demonstrated adequate reliability, validity, and sensitivity to change in observational studies, and some were found to be useful in randomized controlled trials. However, several failed clinical trials have confronted these metrics, as they were not intended for clinical trials. The Outcome Measures Rheumatology group and the US Food and Drug Administration have recommended using measures of disease activity, cumulative organ damage, health-related quality of life, and adverse events as outcomes of interest. Composite responder indices that determine disease global improvement, ensure no significant worsening in unaffected organ systems, and include a physician’s global assessment have been used in randomized clinical trials. Yet unmet therapeutic needs were further challenged by the complex content and psychometric information of the updated instruments, including increased administrative burden associated with demanding training and cost of instruments, and small effect size associated with responsiveness to patient concerns. Nevertheless, with the progress of novel targeted therapy, refining the disease activity metrics is essential. Selection of the disease activity endpoints which is a defining aspect of clinical trial design must be tailored to the outcome of interest and measured by a reliably rated scale characterized by minimal administrative burden. An optimal scale should be simple and practical and incorporate elements of patient concerns.

            Análise do controle por regras

            Na Análise do Comportamento, há uma proposição que sugere que regras não alterariam a probabilidade do comportamento vir a ocorrer no futuro e outra proposição que sugere que regras podem exercer este efeito. O presente estudo tem como objetivo apresentar essas proposições. Antes disso, apresentam-se a distinção entre o comportamento controlado por regras e o controlado por contingências; alguns procedimentos utilizados para investigar o controle por regras e algumas proposições teóricas acerca de como regras funcionam. Em seguida, apresentam-se algumas razões pelas quais regras são seguidas, destacando os efeitos de consequências atrasadas. Conclui-se apresentando argumentos de que regras podem alterar a probabilidade do comportamento vir a ocorrer no futuro.

              Nonadherence to antipsychotics: the role of positive attitudes towards positive symptoms.

              Approximately 50-75% of all patients do not take their antipsychotic medication as prescribed. The current study examined reasons why patients continue versus discontinue antipsychotic medication. We were particularly interested to which extent positive attitudes towards psychotic symptoms foster medication nonadherence. An anonymous online questionnaire was set up to decrease response biases. After a strict selection process, 91 participants with schizophrenia spectrum disorders were retained for the final analyses. On average, 6.2 different reasons for nonadherence were reported. Side-effects (71.4%), sudden subjective symptom improvement (52.4%) and forgetfulness (33.3%) emerged as the most frequent reasons for drug discontinuation. Approximately one fourth of all participants (27.3%) reported at least one positive aspect of psychosis as a reason for nonadherence. In contrast, patients reported on average 3.5 different reasons for adherence (e.g., want to live a normal life (74.6%) and fear of psychotic symptoms (49.3%)). The belief that paranoia represents a survival strategy (subscale derived from the Beliefs about Paranoia Scale) was significantly associated with nonadherence. Patients' attitudes toward medication and the individual illness model need to be carefully considered when prescribing medication. In particular for patients who are likely to discontinue psychopharmacological treatment complementary or alternative psychological treatment should be sought because of a largely increased risk of relapse in the case of sudden drug discontinuation.
