          Abstract

          A fobia social é freqüente entre adolescentes, sendo importante a sua identificação e a determinação de potenciais prejuízos. OBJETIVOS: Relatar a prevalência e o impacto na escolaridade da fobia social em uma amostra de adolescentes. MÉTODOS: O inventário de fobia social (SPIN) foi administrado em 525 alunos dos ensinos fundamental e médio, de ambos os sexos, em uma escola pública e em uma particular, na cidade de Porto Alegre, Brasil, porquanto 32 foram excluídos por respostas incompletas. Foi aplicado também um questionário para identificação das características sociodemográficas da amostra. RESULTADOS: De acordo com o SPIN, 114 dos 493 alunos (23,12%) obtiveram escores iguais ou superiores a 19 pontos no referido inventário, indicando a presença de sintomas compatíveis com o diagnóstico de fobia social. As meninas tenderam a apresentar maior freqüência de transtorno de ansiedade social em relação aos meninos (p = 0,053). Não foi encontrada associação significativa entre repetência e fobia social. CONCLUSÕES: Os sintomas compatíveis com o diagnóstico de fobia social são prevalentes em adolescentes e, em virtude de seu curso crônico, podem causar sérios prejuízos nestes indivíduos. Neste estudo, não foi possível correlacionar fobia social com repetência escolar. No entanto, é de fundamental importância a identificação e o tratamento precoce deste transtorno de ansiedade.

          Social phobia is frequent among adolescents, and its identification and determination of potential impairments are of great importance. OBJECTIVES: To report the prevalence and impact of social phobia on education in a sample of adolescents. METHODS: The Social Phobia Inventory (SPIN) was administered to 525 students of primary and high school, of both genders, in one public and one private school in the city of Porto Alegre, Brazil, but 32 students were excluded from the study due to incomplete answers. A sociodemographic questionnaire were also administered. RESULTS: According to the SPIN, 114 of 493 (23.12%) students obtained scores equal to or greater than 19 points, indicating the presence of symptoms compatible with diagnosis of social phobia. Girls tended to show a greater frequency of social anxiety disorder when compared to boys (p = 0.053). Social phobia was not associated with school repetition. CONCLUSIONS: Social phobia symptoms are common among teenagers and, due to its chronic course, may result in severe impairments. In this study, a correlation between social phobia and school repetition was not found. However, early identification and treatment of such disorder is extremely important.

          The prevalence of DSM-III-R diagnoses in a national sample of Dutch adolescents.

          We estimated the 6-month prevalence of psychiatric disorders among Dutch adolescents, using standardized, internationally available, and replicable assessment procedures, and assessed sex differences and comorbidity of diagnoses. In phase 1, the parent, self-report, and teacher versions of the Child Behavior Checklist screened a sample representative of 13- to 18-year-olds from the Dutch general population. In phase 2, the parent (P) and child (C) versions of the Diagnostic Interview Schedule for Children (DISC) provided DSM-III-R diagnoses for a selected subsample of 780 subjects. The prevalence of any disorder was 21.5% for the DISC-C and 21.8% for the DISC-P. There was little overlap between subjects identified as having a disorder by the DISC-P and the DISC-C; only 4% met the criteria for any disorder on both. The most common disorders were simple phobia, social phobia, and conduct disorder. The most frequent comorbid diagnoses were anxiety and mood disorders. Although prevalences of more than 21% for DISC-C- and DISC-P-derived diagnoses seem high, many adolescents with DSM-III-R diagnoses functioned quite well. The prevalence of any DSM-III-R diagnosis based on the DISC-C or DISC-P, in combination with the criterion for a definite case, was 7.9%.

              Psychometric properties of the Social Phobia Inventory (SPIN): New self-rating scale

              Of available self-rated social phobia scales, none assesses the spectrum of fear, avoidance, and physiological symptoms, all of which are clinically important. Because of this limitation, we developed the Social Phobia Inventory (SPIN). To establish psychometric validation of the SPIN. Subjects from three clinical trials and two control groups were given the 17-item, self-rated SPIN. Validity was assessed against several established measures of social anxiety, global assessments of severity and improvement, and scales assessing physical health and disability. Good test – retest reliability, internal consistency, convergent and divergent validity were obtained. A SPIN score of 19 distinguished between social phobia subjects and controls. The SPIN was responsive to change in symptoms over time and reflected different responses to active drugs v. placebo. Factorial analysis identified five factors. The SPIN demonstrates solid psychometric properties and shows promise as a measurement for the screening of, and treatment response to, social phobia.
