          Abstract

          El liquen plano bucal es una enfermedad inflamatoria crónica cuya etiología permanece desconocida. Se acepta que representa una respuesta inmune mediada por células, en la que existe una interacción linfocito-epitelio, que culmina con la degeneración del estrato basal epitelial. Se ha planteado una alta asociación con enfermedades hepáticas crónicas, como es el caso de la Hepatitis C, no teniendo reportes de estudios similares en nuestro país, por lo que se realizó la detección de de anticuerpos al virus de la Hepatitis C a 51 pacientes de liquen plano bucal procedentes de la consulta de Patología Bucal de la Facultad de Estomatología, atendidos durante el período de 1991-2001 y a 45 controles, mediante una técnica de inmunoensayo con el objetivo de determinar el grado de asociación entre ambas entidades. Encontramos que tres pacientes resultaron positivos y sólo uno del grupo control; sin embargo, la relación entre estas dos enfermedades no se logró establecer, debido probablemente a los bajos índices de seroprevalencia registrados en nuestro país y quizás también al pequeño tamaño de la muestra.

          Translated abstract

          Oral lichen planus is a chronic inflammatory disease of unknown etiology. It is considered that it represents an immune answer mediate by cells in which there exits an interaction lymphocyte-epithelium that ends with the basal layer degeneration. It has been reported a high association with chronic hepatic diseases, as hepatitis C, not having a similar researches reports in our country, that's why we propose ourselves to determine the degree association between both entities. It was made the detection of antibodies to the hepatitis C virus to 51 patients with oral lichen planus, who were assisted in the Department of Oral Pathology of the Dentistry Faculty of Havana Medical Institute since 1991 to 2001 and 45 controls through the immunoassay technique. We found 3 patients positive and only one from the control group. However the relation between these 2 disease couldn't be establish probably due to the low rates of serum prevalence registered in our country and may be because the small sample size.

              Oral lichen planus and malignant transformation: a retrospective follow-up study of clinical and histopathologic data.

              Patients in the stomatology service of the Department of Oral Surgery and Stomatology who were clinically and histopathologically diagnosed with oral lichen planus (OLP) in the years 1995 to 2001 were examined for a possible malignant transformation of a previously biopsied OLP site. For the 145 patients included, the recordings were searched for initial localization and type of OLP lesion, potential noxious agents, distribution between symptomatic and asymptomatic OLP types, and for a malignant transformation of a known OLP site during the follow-up period up to December 2003. The group comprised 47 men and 98 women with a mean age of 56.3 years. Of the 497 lesions, almost half were classified as reticular or papular, predominantly located on the buccal mucosa, gingiva, and borders of the tongue. Four patients did not adhere to their scheduled control visits and were dropped from the study. During the follow-up period 4 patients developed malignant transformation of OLP. In 3 of these cases, dysplasia was present at the initial diagnosis of OLP. This results in a malignant transformation rate of 2.84% among the remaining 141 patients; if the 3 patients with initial dysplasia are excluded, the rate drops to 0.71%. Until further knowledge is derived from large prospective studies, the data supporting or negating a potential malignant character of OLP lesions remains inconclusive. Special emphasis has to be directed toward unified inclusion and exclusion criteria regarding clinical and histologic findings and identifiable risk factors to allow the comparison of different studies.
