          Abstract

The objective of this research is to evaluate the quality of life of patients with laryngeal cancer and indication of total laryngectomy as primary treatment, whereas changes caused by the disease and therapeutic interventions employed. We used a demographic questionnaire and Evaluation Questionnaire for Quality of Life at the University of Washington (UW-QOL). We interviewed 19 patients, mean age was 60 years and 89.5% male. It was found an average composite score of 50.8. The areas that were worst average scores were anxiety, speech and mood. In general, the quality of life of patients is impaired by cancer treatments performed by past and by the expectation of the realization of total laryngectomy, because 47.4% of patients considered their overall quality of life as poor or very poor.

The burden of cancer attributable to alcohol drinking.

We estimated the number of cancer cases and deaths attributable to alcohol drinking in 2002 by sex and WHO subregion, based on relative risks of cancers of the oral cavity, pharynx, esophagus, liver, colon, rectum, larynx and female breast obtained from recent meta- and pooled analyses and data on prevalence of drinkers obtained from the WHO Global Burden of Disease project. A total of 389,100 cases of cancer are attributable to alcohol drinking worldwide, representing 3.6% of all cancers (5.2% in men, 1.7% in women). The corresponding figure for mortality is 232,900 deaths (3.5% of all cancer deaths). This proportion is particularly high among men in Central and Eastern Europe. Among women, breast cancer comprises 60% of alcohol-attributable cancers. Although our estimates are based on simplified assumptions, the burden of alcohol-associated cancer appears to be substantial and needs to be considered when making public health recommendations on alcohol drinking. The impact of head and neck cancer and facial disfigurement on the quality of life of patients and their partners.

Psychological and physical stresses from head and neck cancer can be substantial for patients and partners. There is minimal research exploring treatment impact, particularly facial disfigurement after surgery. Twenty-eight surgery and radiotherapy/brachytherapy/chemoradiation patients and 25 of their partners were compared with 23 radiotherapy/brachytherapy patients and 19 partners. Participants completed the Hospital Anxiety and Depression Scale, Psychosocial Adaptation to Illness Scale, Dyadic Adjustment Scale, and European Organisation for Research and Treatment of Cancer Quality Of Life Scale, including the Head and Neck Cancer module. The Dropkin Disfigurement and Dysfunction scale classified surgical impairment. Partners reported greater distress than patients on some scales. Patients did not have a lower quality of life compared with normal populations and other cancer patients. However, on the EORTC they did and were comparable to a normed sample of head and neck cancer patients. Treatment modality was not predictive of psychological vulnerability. Head and neck cancer patients do not necessarily experience poor quality of life. The disease can have a significant impact on partners. Copyright 2003 Wiley Periodicals, Inc.
