          Abstract

          Resumen El aislamiento del paciente crítico con respecto a su familia en la Unidad de Cuidados Intensivos (UCI), tiene repercusiones para ambos, siendo, fundamental el apoyo emocional de los familiares para la recuperación del paciente. Pudiendo concluir que la UCI es una de las áreas donde más estrés se genera debido a la cantidad de factores estresantes tanto para los pacientes como para sus familiares. A pesar de esto, las necesidades de la familia se desatienden por parte de los enfermeros/as, descuidándose a veces el apoyo emocional que estos familiares necesitan cuando su familiar se encuentra ingresado. Además, la participación de la familia en los cuidados del paciente proporciona apoyo emocional al paciente y disminuye el sufrimiento de ésta, reduciéndose así los síntomas psicológicos que puedan aparecer.

          Translated abstract

          Abstract The isolation of critical patient regarding his family in the Intensive Care Unit (ICU), has implications for both, being fundamental the emotional support of family for patient recovery. Being able to conclude that the UCI is one of the areas where more stress is generated due to the amount of stressful factors for the patients and their family. Despite this, the needs of the family are neglected by nurses, neglecting sometimes the emotional support that these relatives need when their relative is admitted. In addition, the family involvement in patient care provides emotional support to the patient and reduces the suffering of this, thus reducing the psychological symptoms that may appear.

          Writing In and Reading ICU Diaries: Qualitative Study of Families' Experience in the ICU

          Purpose Keeping an ICU patient diary has been reported to benefit the patient's recovery. Here, we investigated the families' experience with reading and writing in patient ICU diaries kept by both the family and the staff. Methods We conducted a qualitative study involving 32 semi-structured in-depth interviews of relatives of 26 patients (34% of all family members who visited patients) who met our ICU-diary criterion, i.e., ventilation for longer than 48 hours. Grounded theory was used to conceptualise the interview data via a three-step coding process (open coding, axial coding, and selective coding). Results Communicative, emotional, and humanising experiences emerged from our data. First, family members used the diaries to access, understand, and assimilate the medical information written in the diaries by staff members, and then to share this information with other family members. Second, the diaries enabled family members to maintain a connection with the patient by documenting their presence and expressing their love and affection. Additionally, families confided in the diaries to maintain hope. Finally, family members felt the diaries humanized the medical staff and patient. Conclusions Our findings indicate positive effects of diaries on family members. The diaries served as a powerful tool to deliver holistic patient- and family-centered care despite the potentially dehumanising ICU environment. The diaries made the family members aware of their valuable role in caring for the patient and enhanced their access to and comprehension of medical information. Diaries may play a major role in improving the well-being of ICU-patient families.

            Multidisciplinary team training to enhance family communication in the ICU.

            Current guidelines from the U.S. Society for Critical Care Medicine state that training in "good communication skills...should become a standard component of medical education and ... available for all ICU caregivers". We sought to train multidisciplinary teams of ICU caregivers in communicating with the families of critically ill patients to improve staff confidence in communicating with families, as well as family satisfaction with their experiences in the ICU.

              The current study investigated the perceived importance and the perceived met needs of family members in the medical intensive care unit and assessed family members' satisfaction with needs met.
