      Rorschach as a Tool in the Psychological Assessment of Families With Patients With Schizophrenia : A Literature Review

          Abstract

          Abstract. Identifying the characteristics of families with offspring who have schizophrenia enables early detection and intervention, thereby reducing the psychiatric damage that may occur. To this end, psychological assessment can be used for both individual and family evaluation, notably the Rorschach. This article aims to review the literature about Conjoint/Consensus Rorschach, as well as articles concerning family, schizophrenia, and Rorschach. Although the Rorschach has been used in this context since the 1960s, it has been employed differently by each researcher. This article suggests further studies on assessing individuals of a family with a member who has schizophrenia using the Rorschach. On the basis of systemic family theories – in which family is understood as a system where members change from interacting with others, in addition to learning relational, behavioral and thought patterns – it can be expected that the psychological assessment of one family member is related to the psychological pattern of the other members. From this psychological evaluation, it is possible to plan adequate treatment models, focusing on finding techniques to cope with anxiety-provoking environments and to teach cognitive strategies in order to reduce the deficits caused by schizophrenia and increase the level of functioning of the subject.

            The Rorschach: Facts, fictions, and future.

            A large body of empirical evidence supports the reliability, validity, and utility of the Rorschach. This same evidence reveals that the recent criticisms of the Rorschach are largely without merit. This article systematically addresses several significant Rorschach components: interrater and temporal consistency reliability, normative data and diversity, methodological issues, specific applications in the evaluation of thought disorder and suicide, meta-analyses, incremental validity, clinician judgment, patterns of use, and clinical utility. Strengths and weaknesses of the test are addressed, and research recommendations are made. This information should give the reader both an appreciation for the substantial, but often overlooked, research basis for the Rorschach and an appreciation of the challenges that lie ahead.

              Early detection and intervention in first-episode schizophrenia: a critical review.

              To review the literature on early intervention in psychosis and to evaluate relevant studies. Early intervention was defined as intervention in the prodromal phase (primary prevention) and intervention after the onset of psychosis, i.e. shortening of duration of untreated psychosis (DUP) (secondary prevention). We found few studies aimed at early intervention, but many papers discussing the idea at a more general level. We identified no studies that prove that intervention in the prodromal phase is possible without a high risk for treating false positives. We identified some studies aimed at reducing DUP, but the results are ambiguous and, until now, no follow-up data showing a positive effect on prognosis have been presented. Early intervention in psychosis is a difficult and important challenge for the psychiatric health services. At the time being reduction of DUP seems to be the most promising strategy. Intervention in the prodromal phase is more ethically and conceptually problematic.
