      Integrative vision in cancer research, prevention and therapy

          Abstract

          There is already a significant global initiative to explore the synergy between traditional medicine (TM) and oncology, for holistic best care of cancer patients. Integrative oncology clinics have emerged with operational efficiency. What is needed now is an integrative vision that inspires to seamlessly coordinate the trans-system efforts in cancer research and rapidly translate the positive outcomes into prevention and treatment of cancer. The current dominant paradigm to consider TM only for complementary and alternative adjunct usage cannot inspire state-of-the art research and development on TM leads and serendipitous discoveries. Ayurvedic concepts of Vyadhi-kshamatwa (Immune resistance), Shatkriyakala (Six stages of a disease) and Hetuviparya Chikitsa (Reversal of pathogenetic factors) need to be synergized with ayurvedic pharmacoepidemiology, reverse pharmacology, observational therapeutics, ayurgenomics, ayurvedic biology, and reverse ayurceutics. Such a paradigm-shifting vision may lead to pragmatic translational research/practice and system obstacles and novel bridges in Integrative Oncology.

          Immune checkpoint targeting in cancer therapy: toward combination strategies with curative potential.

          Research in two fronts has enabled the development of therapies that provide significant benefit to cancer patients. One area stems from a detailed knowledge of mutations that activate or inactivate signaling pathways that drive cancer development. This work triggered the development of targeted therapies that lead to clinical responses in the majority of patients bearing the targeted mutation, although responses are often of limited duration. In the second front are the advances in molecular immunology that unveiled the complexity of the mechanisms regulating cellular immune responses. These developments led to the successful targeting of immune checkpoints to unleash anti-tumor T cell responses, resulting in durable long-lasting responses but only in a fraction of patients. In this Review, we discuss the evolution of research in these two areas and propose that intercrossing them and increasing funding to guide research of combination of agents represent a path forward for the development of curative therapies for the majority of cancer patients.

              Clinical effects of curcumin in enhancing cancer therapy: A systematic review

              Background Curcumin is herbal compound that has been shown to have anti-cancer effects in pre-clinical and clinical studies. The anti-cancer effects of curcumin include inhibiting the carcinogenesis, inhibiting angiogenesis, and inhibiting tumour growth. This study aims to determine the Clinical effects of curcumin in different types of cancers using systematic review approach. Methods A systematic review methodology is adopted for undertaking detailed analysis of the effects of curcumin in cancer therapy. The results presented in this paper is an outcome of extracting the findings of the studies selected from the articles published in international databases including SID, MagIran, IranMedex, IranDoc, Google Scholar, ScienceDirect, Scopus, PubMed and Web of Science (ISI). These databases were thoroughly searched, and the relevant publications were selected based on the plausible keywords, in accordance with the study aims, as follows: prevalence, curcumin, clinical features, cancer. Results The results are derived based on several clinical studies on curcumin consumption with chemotherapy drugs, highlighting that curcumin increases the effectiveness of chemotherapy and radiotherapy which results in improving patient’s survival time, and increasing the expression of anti-metastatic proteins along with reducing their side effects. Conclusion The comprehensive systematic review presented in this paper confirms that curcumin reduces the side effects of chemotherapy or radiotherapy, resulting in improving patients’ quality of life. A number of studies reported that, curcumin has increased patient survival time and decreased tumor markers’ level.
