          Abstract

          A Síndrome de Burnout (SB) consiste em uma resposta a problemas laborais, podendo resultar em adoecimento físico e mental e afastamento laboral. Profissionais da saúde são vulneráveis à SB devido às características do trabalho de cuidado e às formas de gestão. Realizou-se estudo observacional analítico transversal junto a 217 trabalhadores da atenção primária em dois distritos sanitários de Porto Alegre/RS, a fim de conhecer a prevalência e os preditores da SB. Foram aplicados: questionário sociodemográfico e laboral, CESQT (avalia SB perfis 1 (sem culpa) e 2 (com culpa) e SRQ-20 (avalia Transtornos Mentais Comuns - TMC). A análise dos dados compreendeu estatísticas descritivas, teste de Qui-quadrado e análises de regressão linear simples e múltipla. Constatou-se que 18% dos profissionais apresentaram SB perfil 1 e 11% perfil 2. Os preditores para SB perfil 1 foram presença de TMC, cansaço, maior tempo de profissão, não ter filhos, apenas trabalhar, residir em Porto Alegre e trabalhar em UBS. Os preditores para o perfil 2 foram semelhantes, exceto pelas variáveis cidade e tipo de unidade. A elevada prevalência de SB na amostra aponta a necessidade de as instituições de saúde na atenção básica realizarem ações de prevenção e promoção à saúde de seus trabalhadores.

          Translated abstract

          Burnout syndrome (BS) consists in a response to chronic labor problems that becomes from physical and mental illnesses of worker and it may result in absence from work. Health professionals would be a vulnerable category of BS due to the characteristics of care work and management models. An observational analytic cross section was conducted with 217 workers from two primary health districts of Porto Alegre/RS, Brazil, to determine the prevalence and predictors of BS. It was used: demographic and labor questionnaire, SBI (which assesses BS Profile 1 and BS Profile 2) and SRQ - 20 (which assesses common mental disorders - CMD). Data analysis consisted of descriptive statistics, chi-square analysis and simple/multiple linear regression. The results indicated that 18% of professionals present BS profile 1 and 11%, profile 2. Predictors for BS profile 1 were: CMD, fatigue, length of employment, having children, only work, living in Porto Alegre and work at an ordinary primary healthcare unit. For BS profile 2 were similiar, except for the variables: city and kind of local place. The high prevalence of BS in the sample shows the need of institutions of primary healthcare to conduct prevention and promotion health projects among their own workers.

              Psychosocial work environment and mental health--a meta-analytic review.

              To clarify the associations between psychosocial work stressors and mental ill health, a meta-analysis of psychosocial work stressors and common mental disorders was undertaken using longitudinal studies identified through a systematic literature review. The review used a standardized search strategy and strict inclusion and quality criteria in seven databases in 1994-2005. Papers were identified from 24,939 citations covering social determinants of health, 50 relevant papers were identified, 38 fulfilled inclusion criteria, and 11 were suitable for a meta-analysis. The Comprehensive Meta-analysis Programme was used for decision authority, decision latitude, psychological demands, and work social support, components of the job-strain and iso-strain models, and the combination of effort and reward that makes up the effort-reward imbalance model and job insecurity. Cochran's Q statistic assessed the heterogeneity of the results, and the I2 statistic determined any inconsistency between studies. Job strain, low decision latitude, low social support, high psychological demands, effort-reward imbalance, and high job insecurity predicted common mental disorders despite the heterogeneity for psychological demands and social support among men. The strongest effects were found for job strain and effort-reward imbalance. This meta-analysis provides robust consistent evidence that (combinations of) high demands and low decision latitude and (combinations of) high efforts and low rewards are prospective risk factors for common mental disorders and suggests that the psychosocial work environment is important for mental health. The associations are not merely explained by response bias. The impact of work stressors on common mental disorders differs for women and men.
