          Abstract

          A modalidade de atendimento em grupo no hospital é uma forma de trabalho que permite interação e troca entre os participantes, caracterizando-se como espaço de acolhimento às angústias frente a um quadro clínico. A experiência relatada consistiu na realização simultânea de dois grupos, um de pacientes entre 8-15 anos, com quadro de obesidade e sobrepeso, e outro de pais/responsáveis. O objetivo geral foi proporcionar espaço para abordar aspectos emocionais e comportamentais associados ao quadro clínico e estabelecer relações com as dificuldades na adesão ao tratamento. Realizaram-se sete encontros semanais, com atividades que propiciaram reflexão sobre questões como: autoestima, ansiedade, limites e dificuldades na manutenção das dietas e das atividades físicas regulares. A proposta possibilitou compartilhar vivências, debater atitudes, abordar insatisfações e desconfortos e perceber a necessidade de se responsabilizarem pelo tratamento através da mudança de determinados comportamentos. A opção por grupos paralelos permitiu notar mudanças nas relações entre pais/responsáveis e pacientes que contribuíram para melhor resposta ao tratamento. Tal experiência ilustrou a importância de considerar os aspectos emocionais envolvidos na obesidade, favorecendo uma visão mais abrangente do paciente.

          Translated abstract

          The psychological group modality inside the hospital is a way of work that allows interaction and exchange among participants, characterizing itself as a shelter environment to the anguish that is present in a clinical condition. The related experience consisted in the work of two groups: obese patients between 8-15 years old and a group of parents. The main goal of the work groups was to serve as a place to approach the emotional aspects and the challenges in treatment adherence. Seven meetings took place, weekly, with activities that provided space of reflection to matters as: self-esteem, anxiety, limits and difficulties in maintaining the diet and regular physical activity. The proposal made it possible to share experiences, discuss attitudes, address grievances and discomfort and notice the need to take responsibility in the process by changing certain behaviors. The option for parallel groups allowed the notice of changes in relationships between parents and patients, contributing to a better response on the treatment. The experience showed the importance of taking into account the emotional aspects present in obesity, providing a wider vision of the patient.

La propuesta posibilitó compartir vivencias, debatir actitudes, abordar insatisfacciones y faltas de confort y percibir la necesidad de responsabilizarse por el tratamiento a través de la mudanza de determinados comportamientos. La opción por grupos paralelos permitió notar mudanzas en las relaciones entre padres/ responsables y pacientes que contribuyeron para mejor respuesta al tratamiento. Tal experiencia ilustró la importancia de considerar los aspectos emocionales involucrados en la obesidad, favoreciendo una visión más amplia del paciente.

          Effects of television on metabolic rate: potential implications for childhood obesity.

          The effects of television viewing on resting energy expenditure (metabolic rate) in obese and normal-weight children were studied in a laboratory setting. Subjects were 15 obese children and 16 normal-weight children whose ages ranged from 8 to 12 years. All subjects had two measured of resting energy expenditure obtained while at rest and one measurement of energy expenditure taken while viewing television. Results indicated that metabolic rate during television viewing was significantly lower (mean decrease of 211 kcal extrapolated to a day) than during rest. Obese children tended to have a larger decrease, although this difference was not statistically significant (262 kcal/d vs 167 kcal/d, respectively). It was concluded that television viewing has a fairly profound lowering effect of metabolic rate and may be a mechanism for the relationship between obesity and amount of television viewing.

            La globalización y la enseñanza de la psicología en Argentina

            El trabajo aborda tres cuestiones. En primer lugar, analiza brevemente a la historia de la enseñanza de la psicología en Argentina, subrayando la importancia adquirida por el psicoanálisis en la tradición universitaria psicológica. En segundo lugar, desarrolla algunos de los estudios y algunas de las líneas de debate contemporáneo sobre formación universitaria del psicólogo, comenzando por algunos estudios clásicos en los Estados Unidos y concluyendo en la propuesta de diplomatura en psicología, actualmente en debate en los países miembros de la Unión Europea. En tercer lugar, analiza el impacto generado por la globalización en los procesos de formación del psicólogo en Argentina, especialmente a partir del impacto del llamado Mercosur educativo. En ese marco, se analiza críticamente la situación de los programas de psicología en las universidades argentinas.

              The outcome of childhood obesity management depends highly upon patient compliance.

              Over the last 20 years, obesity in childhood and adolescence has become a major public health concern due to dramatically increasing prevalence rates. We evaluated the outcome of 294 children (135 girls, 159 boys) aged 6-16 years (median 10.9 years) enrolled in a single centre outpatient obesity intervention programme consisting of periodical visits to the outpatient unit with regular medical and dietetic counselling aiming at a modification of dietary and activity patterns of patients and parents. The average number of visits to the outpatient unit was 3.6 (SD 2.7) with a mean visit interval of 62.1 days. A mean drop-out rate of 27.5% between each scheduled appointment occurred. In a stepwise regression model, neither reduction of the standard deviation score of patients' body mass index (BMI-SDS) nor dietary counselling contributed to the total number of visits. The only significant contributor was the patients' initial BMI-SDS explaining 4.6% of the variance of attended visits adjusted for age and sex. Segregation of the patients into an "obesity" (initial BMI-SDS >/=90th percentile, at least three visits attended, n=59) and an "excessive obesity" (initial BMI-SDS >/=99th percentile, at least four visits attended, n=75) subgroup showed a mean reduction of BMI-SDS in the "obesity" group at visit three of 0.14 (SD 0.21, P<0.0001), and in the "excessive obese" group at visit four of 0.17 (SD 0.22, P<0.001). In a stepwise regression model, the mean interval between visits and regular dietary counselling explained 11.7% of the variance of BMI-SDS reduction at visit three in both subgroups. there is an urgent need for efficient strategies to improve adherence of outpatients to obesity treatment since, in terms of reduction in body mass index standard deviation score, a beneficial outcome can be achieved for compliant obese children and adolescents.
