      Estimating Health-State Utility Values in Kidney Transplant Recipients and Waiting-List Patients Using the EQ-5D-5L

          Abstract

          Objectives

          To report health-state utility values measured using the five-level EuroQol five-dimensional questionnaire (EQ-5D-5L) in a large sample of patients with end-stage renal disease and to explore how these values vary in relation to patient characteristics and treatment factors.

          Methods

          As part of the prospective observational study entitled “Access to Transplantation and Transplant Outcome Measures,” we captured information on patient characteristics and treatment factors in a cohort of incident kidney transplant recipients and a cohort of prevalent patients on the transplant waiting list in the United Kingdom. We assessed patients’ health status using the EQ-5D-5L and conducted multivariable regression analyses of index scores.

          Results

          EQ-5D-5L responses were available for 512 transplant recipients and 1704 waiting-list patients. Mean index scores were higher in transplant recipients at 6 months after transplant surgery (0.83) compared with patients on the waiting list (0.77). In combined regression analyses, a primary renal diagnosis of diabetes was associated with the largest decrement in utility scores. When separate regression models were fitted to each cohort, female gender and Asian ethnicity were associated with lower utility scores among waiting-list patients but not among transplant recipients. Among waiting-list patients, longer time spent on dialysis was also associated with poorer utility scores. When comorbidities were included, the presence of mental illness resulted in a utility decrement of 0.12 in both cohorts.

          Conclusions

          This study provides new insights into variations in health-state utility values from a single source that can be used to inform cost-effectiveness evaluations in patients with end-stage renal disease.

          EuroQol: the current state of play.

          The EuroQol Group first met in 1987 to test the feasibility of jointly developing a standardised non-disease-specific instrument for describing and valuing health-related quality of life. From the outset the Group has been multi-country, multi-centre, and multi-disciplinary. The EuroQol instrument is intended to complement other forms of quality of life measures, and it has been purposefully developed to generate a cardinal index of health, thus giving it considerable potential for use in economic evaluation. Considerable effort has been invested by the Group in the development and valuation aspects of health status measurement. Earlier work was reported upon in 1990; this paper is a second 'corporate' effort detailing subsequent developments. The concepts underlying the EuroQol framework are explored with particular reference to the generic nature of the instrument. The valuation task is reviewed and some evidence on the methodological requirements for measurement is presented. A number of special issues of considerable interest and concern to the Group are discussed: the modelling of data, the duration of health states and the problems surrounding the state 'dead'. An outline of some of the applications of the EuroQol instrument is presented and a brief commentary on the Group's ongoing programme of work concludes the paper.

            A study of the quality of life and cost-utility of renal transplantation.

            The objective of this study was to assess the cost-utility of renal transplantation compared with dialysis. To accomplish this, a prospective cohort of pre-transplant patients were followed for up to two years after renal transplantation at three University-based Canadian hospitals. A total of 168 patients were followed for an average of 19.5 months after transplantation. Health-related quality of life was assessed using a hemodialysis questionnaire, a transplant questionnaire, the Sickness Impact Profile, and the Time Trade-Off Technique. Fully allocated costs were determined by prospectively recording resource use in all patients. A societal perspective was taken. By six months after transplantation, the mean health-related quality of life scores of almost all measures had improved compared to pre-transplantation, and they stayed improved throughout the two years of follow up. The mean time trade-off score was 0.57 pre-transplant and 0.70 two years after transplantation. The proportion of individuals employed increased from 30% before transplantation to 45% two years after transplantation. Employment prior to transplantation [relative risk (RR) = 23], graft function (RR 10) and age (RR 1.6 for every decrease in age by one decade), independently predicted employment status after transplantation. The cost of pre-transplant care ($66,782 Can 1994) and the cost of the first year after transplantation ($66,290) were similar. Transplantation was considerably less expensive during the second year after transplantation ($27,875). Over the two years, transplantation was both more effective and less costly than dialysis. This was true for all subgroups of patients examined, including patients older than 60 and diabetics. We conclude that renal transplantation was more effective and less costly than dialysis in all subgroups of patients examined.

              Tails from the peak district: adjusted limited dependent variable mixture models of EQ-5D questionnaire health state utility values.

              Health utility data generated by using the EuroQol five-dimensional (EQ-5D) questionnaire are right bounded at 1 with a substantial gap to the next set of observations, left bounded, and multimodal. These features present challenges to the estimation of the effect of clinical and socioeconomic characteristics on health utilities. Our objective was to develop and demonstrate an appropriate method for dealing with these features. We developed a statistical model that incorporates an adjusted limited dependent variable approach to reflect the upper bound and the large gap in feasible EQ-5D questionnaire values. Further flexibility was then gained by adopting a mixture modeling framework to address the multimodality of the EQ-5D questionnaire distribution. We compared the performance of these approaches with that of those frequently adopted in the literature (linear and Tobit models) by using data from a clinical trial of patients with rheumatoid arthritis. We found that three latent classes are appropriate in estimating EQ-5D questionnaire values from function, pain, and sociodemographic factors. Superior performance of the adjusted limited dependent variable mixture model was achieved in terms of Akaike and Bayesian information criteria, root mean square error, and mean absolute error. Unlike other approaches, the adjusted limited dependent variable mixture model fits the data well at high EQ-5D questionnaire levels and cannot predict unfeasible EQ-5D questionnaire values. The distribution of the EQ-5D questionnaire is characterized by features that raise statistical challenges. It is well known that standard approaches do not perform well for this reason. This article developed an appropriate method to reflect these features by combining limited dependent variable and mixture modeling and demonstrated superior performance in a rheumatoid arthritis setting. Further refinement of the general framework and testing in other data sets are warranted. Analysis of utility data should apply methods that recognize the distributional features of the data.
