      Characteristics of the King-Devick test in the assessment of concussed patients in the subacute and later stages after injury

          Abstract

          Although the King-Devick (K-D) test has been used frequently in assessing sports related concussion early after injury, its characteristics over time after injury and in patients with prolonged persistent symptoms are unknown. The purpose of this paper was to: evaluate the ability of the K-D Test to distinguish patients seen early after concussion from those with symptoms persisting more than 3 months compared to controls, assess changes in the K-D test times over time after concussion, and determine the relationship of K-D times to the Stroop Color and Word Test scores. We performed cross-sectional comparisons of patients with recent concussive brain injury (acute group) and those with symptoms persisting more than 3 months to healthy controls on the K-D test, the Sports Concussion Assessment Tool 3 (SCAT3), and the Stroop Color and Word Test. Longitudinal comparisons of the acute group over time within the first month after injury were also made. Post-concussive syndrome (PCS) patients had significantly higher K-D times compared to controls (p = 0.01), while the acute group did not differ from controls(p = 0.33). K-D times at the second visit for the acute group were similar to those of controls (54.7 vs. 49.6, p = 0.31). While SCAT3 scores improved over time in the acute group, the K-D scores did not change between the first and second visit (55.2 vs. 54.7, p = 0.94). K-D scores correlated significantly with the Stroop scores for all three participant groups. The K-D test is likely useful very early after concussion in conjunction with baseline scores, and while scores in PCS patients remain elevated, they can be confounded by factors such as pre-morbid depression and medication use. High correlations with Stroop scores also suggest that performance on the K-D test can by proxy provide additional insight about cognitive function and predict performance on more cognitively demanding tasks.

            Symptomatology and functional outcome in mild traumatic brain injury: results from the prospective TRACK-TBI study.

            Mild Traumatic Brain Injury (mTBI), or concussion, is a major public health concern. There is controversy in the literature regarding the true incidence of postconcussion syndrome (PCS), with the constellation of physical, cognitive, emotional, and sleep symptoms after mTBI. In the current study, we report on the incidence and evolution of PCS symptoms and patient outcomes after mTBI at 3, 6, and 12 months in a large, prospective cohort of mTBI patients. Participants were identified as part of the prospective, multi-center Transforming Research and Clinical Knowledge in Traumatic Brain Injury Study. The study population was mTBI patients (Glasgow Coma Scale score of 13-15) presenting to the emergency department, including patients with a negative head computed tomography discharged to home without admission to hospital; 375 mTBI subjects were included in the analysis. At both 6 and 12 months after mTBI, 82% (n=250 of 305 and n=163 of 199, respectively) of patients reported at least one PCS symptom. Further, 44.5 and 40.3% of patients had significantly reduced Satisfaction With Life scores at 6 and 12 months, respectively. At 3 months after injury, 33% of the mTBI subjects were functionally impaired (Glasgow Outcome Scale-Extended score ≤6); 22.4% of the mTBI subjects available for follow-up were still below full functional status at 1 year after injury. The term "mild" continues to be a misnomer for this patient population and underscores the critical need for evolving classification strategies for TBI for targeted therapy.

              Interdimensional interference in the Stroop effect: uncovering the cognitive and neural anatomy of attention.

              In the classic Stroop effect, naming the color of an incompatible color word (e.g. the word RED printed in green ink; say, 'green') is much slower and more error-prone than is naming the color of a control item (e.g. XXX or CAT printed in green; say 'green'). This seemingly simple interference phenomenon has long provided a fertile testing ground for theories of the cognitive and neural components of selective attention. We present a sketch of the behavioral phenomenon, focusing on the idea that the relative automaticity of the two dimensions determines the direction and the degree of interdimensional interference between them. We then present an outline of current parallel processing explanations that instantiate this automaticity account, and we show how existing interference data are captured by such models. We also consider how Stroop facilitation (faster response of 'red' to RED printed in red) can be understood. Along the way, we describe research on two tasks that have emerged from the basic Stroop phenomenon - negative priming and the emotional Stroop task. Finally, we provide a survey of brain imaging research, highlighting the possible roles of the anterior cingulate in maintaining attentional set and in processing conflict or competition situations.
