          Abstract

          En este tema tratamos las picaduras por artrópodos. Se hace referencia a las diferencias que existen entre las picaduras de avispas y abejas, comentando la composición de venenos y las diferentes reacciones locales y generales que provocan dichas picaduras. Se exponen además las picaduras-mordeduras producidas por escorpiones, arañas, garrapatas, y animales marinos con la clínica que provocan y el tratamiento que es necesario administrar. Por último, se incluyen las mordeduras por serpientes, haciendo referencia a los tipos de ofidios más frecuentes en Navarra, la forma de diferenciar la mordedura de culebras de las víboras, la diferente clínica que provocan, y el tratamiento a aplicar.

          Translated abstract

          Under the heading of this subject we deal with stings by arthropods, making reference to the differences that exist between the stings of wasps and bees, commenting on the composition of the poisons and the different local and general reactions that are caused by such stings. Also discussed are the stings/bites caused by scorpions, spiders, ticks, and marine animals, with the clinical picture they provoke and the treatment that must be administered. Finally, snakebites are considered, with reference to the most frequent types of ophidia to be found in Navarra, how to differentiate between the bites of snakes and vipers, the different clinical pictures they provoke and the treatment to be applied

          Epidemiology of Hymenoptera allergy.

          Epidemiology and especially the natural history of Hymenoptera allergy form the background that is essential to improving the clinical management of insect venom allergic patients. This review focuses on the emergence of recent data which could help provide further enlightenment in this field. The latest data on the extent of the disease, the risk factors for sensitization and for local, systemic and fatal reactions after a hymenoptera sting are reviewed. The emerging problems concerning asymptomatic sensitization, the meaning of constitutively elevated tryptase serum levels and the persisting widespread poor awareness of available therapies in Hymenoptera allergic patients are particularly emphasized. The assessment of the risk for systemic reaction in skin-positive subjects with a negative case history, and the suggestion of the baseline serum tryptase level as a risk factor for reaction severity after a sting, are the most important clinical implications of the latest studies. The genetic and environmental factors involved in the persistence of venom specific immunoglobulin E after a sting and the factors which orient towards a systemic or a large local reaction after apparently the same sting remain open questions.

              Ocular lesions arising after stings by hymenopteran insects.

              To describe the ocular lesions that occur after stings from hymenopteran insects. We examined the ocular alterations in five patients who suffered ocular trauma from hymenopteran insect stings. In 4 cases where the insect was identified as a wasp, all the patients presented with persistent corneal decompensation and two presented with total cataract, requiring surgical treatment. In the case of trauma by a bee sting, the patient presented with corneal edema and an inflammatory reaction of the anterior chamber with total regression of these alterations after clinical treatment. Although a rare occurrence, ocular trauma caused by hymenopteran insects can result in severe ocular alterations in humans.
