          Abstract

          Resumen Objetivos: El objetivo general es establecer qué tipo de cura, la cura en ambiente húmedo o la terapia de presión negativa, es más eficiente para el tratamiento de las lesiones por presión. También, identificar los índices de coste-efectividad, contrastar los resultados obtenidos al aplicar los 2 tipos de curas en las diferentes categorías de lesiones por presión, determinar los conocimientos y percepción de enfermería en el ámbito de las lesiones por presión, y comparar las sensaciones experimentadas por los pacientes al recibir las curas. Metodología: Se realizó un estudio de coste a partir de una revisión bibliográfica en las siguientes bases de datos: SciELO, Google Académico, CUIDEN, IBECS, Elsevier, PubMed, MEDLINE, Dialnet, Scopus y CINAHL, durante los meses de septiembre a diciembre de 2020. Resultados: Se obtuvieron un total de 31 artículos que hacen referencia a la cura en ambiente húmedo y/o a la terapia de presión negativa aplicadas en lesiones por presión de diferentes categorías. Dichos documentos están comprendidos entre 2010 y 2020, ambos incluidos, muestran el texto completo y están publicados en español o inglés. El 82% de los documentos analizan el índice coste-efectividad, teniendo en cuenta el gasto final de tratamiento y los resultados de cada tipo de cura, entre otras variables. Conclusiones: El tratamiento de elección para las lesiones por presión categorías I y II es la cura en ambiente húmedo, mientras que en las lesiones por presión categorías III y IV, la evidencia científica actual muestra mejores resultados con terapia de presión negativa.

          Translated abstract

          Abstract Objectives: The general objective is to establish which type of cure, the moist wound healing (MWH) or the negative pressure wound therapy (NPWT), is more efficient for the treatment of pressure injuries (PI). Also, to identify the cost-effectiveness indices, to compare the results obtained by applying the two types of treatment in the different categories of PI, to determine the knowledge and perception of nursing in the field of PI and to compare the sensations experienced by patients when receiving treatment. Methodology: A cost study was made based on a literature review in the following databases: SciELO, Google Scholar, CUIDEN, IBECS, Elsevier, PubMed, MEDLINE, Dialnet, Scopus and CINAHL, during the months of September to December 2020. Results: A total of 31 articles were obtained that refer to MWH and/or NPWT applied in PI of different categories. These documents are from 2010 to 2020 both inclusive, show full text and are published in English or Spanish. 82% of the documents analyse the cost-effectiveness index, including the final cost of treatment and the results of each type of cure, among other variables. Conclusions: The treatment of choice for PI categories I and II is MWH, while in PI categories III and IV the current scientific evidence shows better results with NPWT.

          Pressure ulcers: Current understanding and newer modalities of treatment

          This article reviews the mechanism, symptoms, causes, severity, diagnosis, prevention and present recommendations for surgical as well as non-surgical management of pressure ulcers. Particular focus has been placed on the current understandings and the newer modalities for the treatment of pressure ulcers. The paper also covers the role of nutrition and pressure-release devices such as cushions and mattresses as a part of the treatment algorithm for preventing and quick healing process of these wounds. Pressure ulcers develop primarily from pressure and shear; are progressive in nature and most frequently found in bedridden, chair bound or immobile people. They often develop in people who have been hospitalised for a long time generally for a different problem and increase the overall time as well as cost of hospitalisation that have detrimental effects on patient's quality of life. Loss of sensation compounds the problem manifold, and failure of reactive hyperaemia cycle of the pressure prone area remains the most important aetiopathology. Pressure ulcers are largely preventable in nature, and their management depends on their severity. The available literature about severity of pressure ulcers, their classification and medical care protocols have been described in this paper. The present treatment options include various approaches of cleaning the wound, debridement, optimised dressings, role of antibiotics and reconstructive surgery. The newer treatment options such as negative pressure wound therapy, hyperbaric oxygen therapy, cell therapy have been discussed, and the advantages and disadvantages of current and newer methods have also been described.

            Una aproximación al impacto del coste económico del tratamiento de las úlceras por presión en España

            Las úlceras de presión son un importante problema de salud en todas las escenas del cuidado y tiene un gran impacto en los pacientes y en los recursos del Sistema de Salud y socio-sanitario. El objetivo de este estudio es el de estimar el coste del tratamiento global de las úlceras por presión en España usando para ello información proveniente de diferentes fuentes de datos primarias. Este estudio demuestra que el costo de tratamiento de una úlcera de presión aumenta substancialmente en razón directa con la severidad de la úlcera, yendo desde los 24 € ([\(32.16], Grado I) a los 6.802 € ([\) 9.115], Grado IV) para pacientes tratados en hospitales. La severidad de la úlcera aumenta el costo global porque el tiempo de curación es más prolongado y la incidencia de complicaciones más alta en los casos más severos. El costo total de tratamiento de las úlceras por presión en España en un año es aproximadamente de 461 millones de euros ([$ 618 millones], cerca del 5% del gasto sanitario anual). De este montante, el 15% lo representan el costo de apósitos y otros materiales, mientras que el 19% lo representan el costo del tiempo de enfermería, y el 45% del total lo representa el costo de las estancias extra en el hospital relacionadas con estas lesiones.

              Negative pressure therapy for the treatment of complex wounds

              ABSTRACT The objective of this study is to evaluate the effectiveness of negative pressure therapy (NPT) in the treatment of complex wounds, with emphasis on its mechanisms of action and main therapeutic indications. We searched the Pubmed / Medline database for articles published from 1997 to 2016, and selected the most relevant ones. The mechanisms of action of NPT involveboth physical effects, such as increased perfusion, control of edema and exudate, reduction of wound dimensions and bacterial clearance, and biological ones, such as the stimulation of granulation tissue formation, microdeformations and reduction of Inflammatory response. The main indications of NPT are complex wounds, such as pressure ulcers, traumatic wounds, operative wound dehiscences, burns, necrotizing wounds, venous ulcers, diabetic wounds, skin grafts, open abdomen, prevention of complications in closed incisions and in the association with instillation of solutions in infected wounds.
