      Changing pattern of utilization of human donor cornea in India

          Abstract

          Purpose:

          To review the changing pattern of donor, corneal utilization in an eye bank at a Tertiary Care Center in Northern India by analyzing the trend in the years 2003, 2008, and 2011.

          Methods:

          A retrospective review of eye bank records for 3 years (2003, 2008, and 2011) was performed at the National Eye Bank. Details including a clinical grade of donor cornea, indication of corneal transplantation (therapeutic or optical), type of procedure (penetrating or lamellar keratoplasty [LK]), and clinical diagnosis of the graft recipients were recorded. Primary outcome measure was to observe any preference toward LK, judicious usage of donor corneal tissue, and impact of lamellar corneal transplant in the usage of donor corneas. Secondary outcomes included overall utilization rate and change in trend of indication for keratoplasty.

          Results:

          A total of 673, 745, and 864 corneas were retrieved in the years 2003, 2008, and 2011, respectively. The percentage of donor corneal utilization increased significantly over time with the rate being 65.08%, 70.06%, and 68.29%, respectively, in the years 2003, 2008, and 2011 ( P = 0.014); however, this change was reflected only in the usage of nonoptical grade corneas and not for the optical grade corneas. There was an overall increase in lamellar corneal procedures for any clinical grade of cornea ( P = 0.0019); number of Descemet's stripping automated endothelial keratoplasty (DSAEK) procedures increased significantly ( P < 0.001), particularly for pseudophakic corneal edema (PCE) ( P = 0.0085) and failed graft ( P = 0.002). Significant increase in the utilization of nonoptical grade corneas was observed over the years ( P = 0.005), though the utilization did not increase significantly for optical purposes viz., LK ( P = 0.08).

          Conclusions:

          Utilization rate of donor corneas increased over the years, primarily due to increase in usage of nonoptical grade corneas for therapeutic purposes. There was a procedural shift toward DSAEK for PCE and failed graft. However, an increase in usage of nonoptical grade corneas for LK, a single donor corneal tissue for two recipients, and retrieval or utilization of optical grade cornea was not observed.

          Descemet's stripping endothelial keratoplasty: safety and outcomes: a report by the American Academy of Ophthalmology.

          To review the published literature on safety and outcomes of Descemet's stripping endothelial keratoplasty (DSEK) for the surgical treatment of endothelial diseases of the cornea. Peer-reviewed literature searches were conducted in PubMed and the Cochrane Library with the most recent search in February 2009. The searches yielded 2118 citations in English-language journals. The abstracts of these articles were reviewed and 131 articles were selected for possible clinical relevance, of which 34 were determined to be relevant to the assessment objectives. The most common complications from DSEK among reviewed reports included posterior graft dislocations (mean, 14%; range, 0%-82%), followed by endothelial graft rejection (mean, 10%; range, 0%-45%), primary graft failure (mean, 5%; range, 0%-29%), and iatrogenic glaucoma (mean, 3%; range, 0%-15%). Average endothelial cell loss as measured by specular microscopy ranged from 25% to 54%, with an average cell loss of 37% at 6 months, and from 24% to 61%, with an average cell loss of 42% at 12 months. The average best-corrected Snellen visual acuity (mean, 9 months; range, 3-21 months) ranged from 20/34 to 20/66. A review of postoperative refractive results found induced hyperopia ranging from 0.7 to 1.5 diopters (D; mean, 1.1 D), with minimal induced astigmatism ranging from -0.4 to 0.6 D and a mean refractive shift of 0.11 D. A review of graft survival found that clear grafts at 1 year ranged from 55% to 100% (mean, 94%). The evidence reviewed is supportive of DSEK being a safe and effective treatment for endothelial diseases of the cornea. In terms of surgical risks, complication rates, graft survival (clarity), visual acuity, and endothelial cell loss, DSEK appears similar to penetrating keratoplasty (PK). It seems to be superior to PK in terms of earlier visual recovery, refractive stability, postoperative refractive outcomes, wound and suture-related complications, and intraoperative and late suprachoroidal hemorrhage risk. The most common complications of DSEK do not appear to be detrimental to the ultimate vision recovery in most cases. Long-term endothelial cell survival and the risk of late endothelial rejection are beyond the scope of this assessment. Proprietary or commercial disclosure may be found after the references.

            Big-bubble technique to bare Descemet's membrane in anterior lamellar keratoplasty.

            We describe a lamellar keratoplasty technique to bare Descemet's membrane in which air is injected to detach the central Descemet's. After a partial-thickness corneal trephination is performed, a disposable needle is inserted, deeply and bevel down, into the paracentral corneal stroma and air is injected. In most cases, this forms a large air bubble between Descemet's membrane and the corneal stroma. After anterior lamellar keratectomy is performed, a small opening is made in the air bubble and the remaining stromal layers are lifted with an iris spatula, severed with a blade, and excised with scissors. This technique is faster, safer, and easier to perform than previous methods.

              Comparison of posterior lamellar keratoplasty techniques to penetrating keratoplasty.

              To describe the visual outcomes and intraoperative and postoperative complications after penetrating keratoplasty (PK), deep lamellar endothelial keratoplasty (DLEK), Descemet stripping endothelial keratoplasty (DSEK), and Descemet stripping automated endothelial keratoplasty (DSAEK) and to compare the results with those of previously reported series. Prospective, comparative, nonrandomized study. One hundred seventy-seven eyes of 161 consecutive patients who had corneal edema resulting from Fuchs endothelial dystrophy, pseudophakic bullous keratopathy, aphakic bullous keratopathy, failed graft or iridocorneal endothelial syndrome. All patients underwent either PK, DLEK, DSEK, or DSAEK at the Cornea Service of the Toronto Western Hospital. Uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity (BSCVA), manifest refraction, corneal endothelial counts, and postoperative complications. The average 12-month postoperative BSCVA was 20/53 in the PK group, 20/80 in the DLEK group, 20/56 in the DSEK group, and 20/44 in the DSAEK group. The mean spherical equivalent was similar between groups, but tended toward hyperopia in the DSEK and DSAEK groups. The average refractive astigmatism was 3.78+/-1.91 diopters (D) in the PK group and 1.61+/-1.26 D, 1.86+/-1.1 D, and 1.36+/-0.92 D in the DLEK, DSEK, and DSAEK groups, respectively (P<0.0001). Early postoperative donor disc dislocations occurred in 6 (8.8%) patients in the DLEK group, 2 (12.5%) in the DSEK group, and 7 (15.6%) in the DSAEK group (P = 0.0004). Detached grafts were reattached and repositioned by injecting an air bubble to press the donor against the recipient cornea. Primary graft failure occurred in 1 (2.1%) of the PK cases, 2 (2.9%) of the DLEK cases, and 1 (2.2%) of the DSAEK cases; all underwent the same procedure successfully. Average cell loss at 1 year after surgery was 40.11% and was similar in the 4 groups. The DSAEK surgery enabled rapid and better UCVA and BSCVA when compared with PK, DLEK, and DSEK, with significantly lower astigmatism. Endothelial cell loss was similar, but the dislocation rate was significantly higher in the DSAEK group.
