      Probiotics for the Control of Helminth Zoonosis

          Abstract

          This paper is a comprehensive, concise, and an up to date review about probiotics effect and mechanisms against helminth infections of zoonotic importance. Zoonoses are diseases that can be transmitted from animals to humans in a reversible way. Despite zoonotic helminth diseases being still a challenge to the public health and the agriculture industries globally, they were still neglected in both human and veterinary medicine. Moreover, the increasing emergence of anthelmintic drug resistance constitutes failures of most disease control strategies, alarming for a quest to new alternative control approaches. Consequently, the use of beneficial microorganisms, probiotics, is becoming interesting for its prophylactic or therapeutic application against several diseases including helminths. Recent studies on probiotics against parasites and the interactions between bacteria, parasites, and the immune system in the gut draw much attention. However, the effects of these beneficial microorganisms in helminth infections remain largely unexplored. Therefore, the aim of the present review is to raise attention and to summarize recent findings on probiotics research against helminth parasites of zoonotic significance. State-of-the-art research on beneficial effects of bacteria on helminth infections and their proposed mechanisms of action is thoroughly discussed.

          Trichuris suis therapy in Crohn's disease.

          Crohn's disease is common in highly industrialised Western countries where helminths are rare and uncommon in less developed areas of the world where most people carry worms. Helminths diminish immune responsiveness in naturally colonised humans and reduce inflammation in experimental colitis. Thus exposure to helminths may help prevent or even ameliorate Crohn's disease. The aim of the study was to determine the safety and possible efficacy of the intestinal helminth Trichuris suis in the treatment of patients with active Crohn's disease. Twenty nine patients with active Crohn's disease, defined by a Crohn's disease activity index (CDAI) > or =220 were enrolled in this open label study. All patients ingested 2500 live T suis ova every three weeks for 24 weeks, and disease activity was monitored by CDAI. Remission was defined as a decrease in CDAI to less than 150 while a response was defined as a decrease in CDAI of greater than 100. At week 24, 23 patients (79.3%) responded (decrease in CDAI >100 points or CDAI <150) and 21/29 (72.4%) remitted (CDAI <150). Mean CDAI of responders decreased 177.1 points below baseline. Analysis at week 12 yielded similar results. There were no adverse events. This new therapy may offer a unique, safe, and efficacious alternative for Crohn's disease management. These findings also support the premise that natural exposure to helminths such as T suis affords protection from immunological diseases like Crohn's disease.

            The socioeconomic burden of parasitic zoonoses: global trends.

            Diseases resulting from zoonotic transmission of parasites are common. Humans become infected through food, water, soil and close contact with animals. Most parasitic zoonoses are neglected diseases despite causing a considerable global burden of ill health in humans and having a substantial financial burden on livestock industries. This review aims to bring together the current data available on global burden estimates of parasitic zoonoses and indicate any changes in the trends of these diseases. There is a clear need of such information as interventions to control zoonoses are often in their animal hosts. The costs of such interventions together with animal health issues will drive the cost effectiveness of intervention strategies. What is apparent is that collectively, parasitic zoonoses probably have a similar human disease burden to any one of the big three human infectious diseases: malaria, tuberculosis or HIV in addition to animal health burden. Although the global burden for most parasitic zoonoses is not yet known, the major contributors to the global burden of parasitic zoonoses are toxoplasmosis, food borne trematode infections, cysticercosis, echinococcosis, leishmaniosis and zoonotic schistosomosis.               Probiotics, gut microbiota and health.

              The human gut is a huge complex ecosystem where microbiota, nutrients, and host cells interact extensively, a process crucial for the gut homeostasis and host development with a real partnership. The various bacterial communities that make up the gut microbiota have many functions including metabolic, barrier effect, and trophic functions. Hence, any dysbiosis could have negative consequences in terms of health and many diseases have been associated to impairment of the gut microbiota. These close relationships between gut microbiota, health, and disease, have led to great interest in using probiotics (i.e. live micro-organisms), or prebiotics (i.e. non-digestible substrates) to positively modulate the gut microbiota to prevent or treat some diseases. This review focuses on probiotics, their mechanisms of action, safety, and major health benefits. Health benefits remain to be proven in some indications, and further studies on the best strain(s), dose, and algorithm of administration to be used are needed. Nevertheless, probiotic administration seems to have a great potential in terms of health that justifies more research.
