          Abstract

          Las unidades de cuidados intensivos deben estar preparadas para un eventual desastre, bien sea de índole interno o externo, donde sea necesario evacuar a los pacientes ingresados. Deben disponer de un Plan de Emergencia y Autoprotección donde estén recogidos los criterios de evacuación de los pacientes, que debe conocer todo el personal que trabaja en el servicio. Para esto, habrá que hacer un triaje a los pacientes basándose en las prioridades de atención y según las posibilidades de supervivencia. El disponer de un plan de evacuación, conocido por todo el personal y actualizado mediante la realización de simulacros periódicos, debería incluirse como un indicador de calidad que se debería cumplir, ya que con esto se conseguiría una mejor atención al paciente en caso de presentarse una situación de desastre donde hubiere que evacuar la unidad de cuidados intensivos.

          Translated abstract

          The intensive care units must be prepared for a possible disaster, whether internal or external, in case it becomes necessary to evacuate the in-patients. They must have an Emergency and Self-protection Plan that includes the patient evacuation criteria and this must be known by all the personnel who work in the service. For that reason, the patients must be triaged, based on their attention priorities, according to their survival possibilities. Having an evacuation, known by all the personnel and updated by means of the performance of periodic drills, should be included as a quality indicator that must be met, since this would achieve better attention to the patient in case of a disaster situation requiring the evacuation of the ICU.

          Recomendaciones de tratamiento al final de la vida del paciente crítico

          El ingreso de un paciente en la Unidad de Medicina Intensiva (UMI) está justificado cuando es posible revertir o paliar su situación crítica; a pesar de ello, en las UMI se produce una alta mortalidad con respecto a otros servicios hospitalarios. El tratamiento al final de la vida de los pacientes críticos y la atención a las necesidades de sus familiares están lejos de ser adecuados, por diversos motivos: la sociedad niega u oculta la muerte, es muy difícil predecirla con exactitud, con frecuencia el tratamiento está fragmentado entre diferentes especialistas y hay una insuficiente formación en medicina paliativa, incluyendo habilidades de comunicación. Se producen frecuentes conflictos relacionados con las decisiones que se toman en torno a los enfermos críticos que están en el final de su vida, especialmente con la limitación de tratamientos de soporte vital (LTSV). La mayoría son conflictos de valores entre las diversas partes implicadas: el paciente, sus familiares y/o representantes, los profesionales sanitarios y la institución. El Grupo de trabajo de Bioética de la SEMICYUC elabora estas Recomendaciones de tratamiento al final de la vida del paciente crítico con el propósito de contribuir a la mejora de nuestra práctica diaria en tan difícil campo. Tras el análisis del papel de los agentes implicados en la toma de decisiones (pacientes, familiares, profesionales e instituciones sanitarias) y de los fundamentos éticos y legales de la omisión y retirada de tratamientos, se aconsejan unas pautas de actuación en lo referente a la sedación en el final de la vida y la retirada de la ventilación mecánica, se matiza el papel de las instrucciones previas en medicina intensiva y se propone un formulario que refleje de forma escrita las decisiones adoptadas.

            Analysis of disaster response plans and the aftermath of Hurricane Katrina: lessons learned from a level I trauma center.

            The purpose of this study was to compare disaster preparedness of a Level I Trauma Center with performance in an actual disaster. Previous disaster response evaluations have shown that the key to succeeding in responding to a catastrophic event is to anticipate the event, plan the response, and practice the plan. The Emergency Management Team had identified natural disaster as the hospital's highest threat. The hospital also served as the regional hospital for the Louisiana Health Resources and Service Administration Bioterrorism Hospital Preparedness Program. The hospital master disaster plan, including the Code Gray annex, was retrospectively reviewed and compared with the actual events that occurred after Hurricane Katrina. Vital support areas were evaluated for adequacy using a systematic approach. In addition, a survey of 10 key personnel from trauma and emergency medicine present during Hurricane Katrina was conducted. The survey of vital support areas were scored as adequate (3 pts), partially adequate (2 pts), or inadequate (1 pt). Ninety-three percent of the line items on the Code Gray Checklist were accomplished before landfall of the storm. The results of the survey of vital support areas were water-3.0, food-2.4, sanitation-1.5, communication-1.4, and power-1.5. Despite identifying the threat of a major hurricane, preparing a response plan, and exercising the plan, a major medical center can be overwhelmed by a catastrophic disaster like Hurricane Katrina. We offer our lessons-learned as an aid for other medical centers that are developing and exercising their plans.
