      Physical inactivity among physiotherapy undergraduates: exploring the knowledge-practice gap

          Abstract

          Background

          Physical inactivity is a common risk factor for several non-communicable diseases (NCDs). Increasing physical activity could reduce the burden of disease due to major NCDs and increase life expectancy. Undergraduate physiotherapy students represent a group of young-adults expected to have a good knowledge of physical activity. We evaluated physical activity levels of undergraduate physiotherapy students of University of Colombo, Sri Lanka and determined their motives and barriers for participation in physical activity.

          Methods

          All physiotherapy undergraduates studying at the University of Colombo, Sri Lanka in 2013 were invited for the study. Phase one was a quantitative study to evaluate the physical activity levels and phase two was a qualitative study to identify motives and barriers for physical activity and sports in the same cohort. Physical activity levels (phase 1) were assessed using the interviewer administered International Physical Activity Questionnaire (long-version). The qualitative study (phase 2) was conducted in the same population using Focus Group Discussions ( n = 3) and individual In-depth Interviews ( n = 5).

          Results

          Sample size in phase 1 and phase 2 were 113 (response rate = 98%; [N-115]) and 87 (response rat = 97%; [N-90]) respectively. Mean age (±SD) of participants was 23.4 ± 1 years. The mean weekly total MET minutes (±SD) of the study population was 1791.25 ± 3097. According to the IPAQ categorical score a higher percentage of participants were ‘inactive’ (48.7%), while only 15.9% were in the ‘Highly active’ group. Lack of support and encouragement received during childhood to engage in sports activity seem to have played an important role in continuing their exercise behavior through to the adult life. Academic activities were given priority by both parents and teachers. The environment and support from teachers, family and friends were important to initiate and adhere to sports and physical activity.

          Conclusions

          A higher percentage of participants were ‘inactive’, in spite of belonging to a group which is presumed to be knowledgeable regarding the benefits of physical activity. A significant negative attitude towards physical activity was observed in this cohort of young-adults. This seems to stem from earlier in life, due to lack of support and motivation for physical exercise and sports, received during primary and secondary schooling. This negative attitude has become a significant ‘internal’ barrier, which has not been changed in spite of their education.

          Physical activity from childhood to adulthood: a 21-year tracking study.

          The aim of this study was to investigate stability of physical activity from childhood and adolescence to adulthood in multiple age cohorts, and analyze how well adult physical activity can be predicted by various physical activity variables measured in childhood and adolescence. The data were drawn from the Cardiovascular Risk in Young Finns Study. The study was started in 1980, when cohorts of randomly sampled boys and girls aged 3, 6, 9, 12, 15, and 18 years (total of 2309 subjects) were examined for the first time. The measurements were repeated in 1983, 1986, 1989, 1992, and 2001. In 2001, the subjects (n =1563, 68%) were aged 24, 27, 30, 33, 36, and 39 years, respectively. Physical activity was measured by means of a short self-report questionnaire that was administered individually in connection with a medical examination. On the basis of a questionnaire, a physical activity index (PAI) was calculated. There were no significant differences in the 1980 PAI between participants and dropouts in 2001. Spearmans rank order correlation coefficients for the 21-year tracking period varied from 0.33 to 0.44 in males, and from 0.14 to 0.26 in females. At shorter time intervals the correlation was higher. On average, the tracking correlation was lower in females than in males. Persistent physical activity, defined as a score in the most active third of the PAI in two or three consecutive measurements, increased the odds that an individual would be active in adulthood. Odds ratios for 3-year continuous activity versus continuous inactivity varied from 4.30 to 7.10 in males and 2.90 to 5.60 in females. The corresponding odds ratios for 6-year persistence were 8.70 to 10.80 and 5.90 to 9.40. It was concluded that a high level of physical activity at ages 9 to 18, especially when continuous, significantly predicted a high level of adult physical activity. Although the correlations were low or moderate, we consider it important that school-age physical activity appears to influence adult physical activity, and through it, the public health of the general population.

            Translating social ecological theory into guidelines for community health promotion.

            Health promotion programs often lack a clearly specified theoretical foundation or are based on narrowly conceived conceptual models. For example, lifestyle modification programs typically emphasize individually focused behavior change strategies, while neglecting the environmental underpinnings of health and illness. This article compares three distinct, yet complementary, theoretical perspectives on health promotion: behavioral change, environmental enhancement, and social ecological models. Key strengths and limitations of each perspective are examined, and core principles of social ecological theory are used to derive practical guidelines for designing and evaluating community health promotion programs. Directions for future health promotion research are discussed, including studies examining the role of intermediaries (e.g., corporate decision-makers, legislators) in promoting the well-being of others, and those evaluating the duration and scope of intervention outcomes.

              Addressing overreporting on the International Physical Activity Questionnaire (IPAQ) telephone survey with a population sample.

              To examine a possible problem of overreporting and to describe the degree of error with the International Physical Activity Questionnaire (IPAQ) short telephone protocol. Cross-sectional study, using two different physical activity (PA) self-report protocols. Telephone interviews about PA in Belgium. Fifty adults who had previously been interviewed with IPAQ in a national survey. Seventy-five per cent reported less PA with the modified procedure than with the IPAQ. Twenty-three of the 50 individuals were found to have reported some amounts of PA with the IPAQ (either walking, or vigorous or moderate PA) when they should have reported none. In total, based on their revised reports of PA, 50% fewer persons met PA recommendations than was the case with IPAQ. The overreporting could not be related to types of error-prone individuals. Overreporting of PA in population samples is a serious problem that could be reduced by implementing procedure changes without changing the IPAQ items themselves.
