      Effect of Lateral Single Bar Knee Orthoses in Correction of Genu Varum in Nutritional Rickets: A Randomized Trial

          ABSTRACT

          Introduction

          Genu varum is a common pediatric knee deformity and seen as a physiological variation in the growth pattern. Rickets causes more pathological bowing if occurs during the phase of physiological bowing. Apart from vitamin D restoration, the role of orthosis in the correction of the genu varum due to rickets is not clear. This study is done to compare the effect of vitamin D restoration with and without orthosis in correction of the genu varum in nutritional rickets.

          Materials and methods

          Sixty children with a bilateral genu varum were enrolled as per inclusion and exclusion criteria and divided into two equal groups. Group I received the Stoss therapy for vitamin D restoration with bilateral lateral single bar knee orthoses, and group II received the Stoss therapy only. The intercondylar distance and radiographic tibiofemoral angle were measured at the beginning and for three follow-up visits at 2 monthly intervals.

          Results

          The intercondylar distance and tibiofemoral angle decreased significantly ( p ≤0.001) at subsequent follow-ups in each group. The percentage change in the intercondylar distance and the tibiofemoral angle at the subsequent follow-up on comparison in both groups was statistically insignificant.

          Conclusion

          Lateral single bar knee orthoses do not provide any additional benefit over vitamin D restoration by the Stoss therapy for the treatment of the genu varum in nutritional rickets.

          Biomechanical effect of unloader braces for medial osteoarthritis of the knee: a systematic review (CRD 42015026136)

          Purpose There is a lack of consensus regarding biomechanical effects of unloader braces for the treatment of medial osteoarthritis (OA) of the knee. The purpose of this study was to perform a systematic review of studies examining the biomechanical effect of unloader braces. Methods A systematic search for articles about the biomechanical effect of unloader braces was performed. Primary outcome measure was the influence of the brace on the knee adduction moment. Data sources were Pubmed central and google scholar. Results Twenty-four articles were included. Twenty articles showed that valgus unloader braces significantly decrease the knee adduction moment. Seven of those studies reported a decrease of pain in braced patients (secondary outcome measure). Positive effects on the knee adduction moment could be found for custom made braces for conventional knee braces and for a foot ankle orthosis. Four studies could not show any effect of knee unloader braces on the knee adduction moment although one of these studies found decreased pain in braced patients. One of these studies examined healthy patients with a neutral axis. Conclusion This systematic review could demonstrate evidence that unloader braces reduce the adduction moment of the knee. Foresighted, a systematic review about the clinical effect of unloader braces is required.

            Orthotic management of severe genu varum and tibia vara.

            In two cases of severe genu varum and one case of infantile tibia vara, we employed double-upright knee-ankle-foot orthoses with free knee and ankle joints. The patients tolerated these orthoses well and were able to walk. The results were satisfactory.
