      Six Years of Sick Leave Spells in a Group of University Civil Workers. Can Modern Work Bring Them a New Health Problem?

          Abstract

          The objective of this study is to analyse sick leave episodes of a university’s collective of statutory workers in the State of São Paulo, between January 2010 and December 2015. For this, a descriptive study analysed 5776 registered spells of sick leave of four university units: agricultural sciences; human health, health and animal reproduction, and biological sciences; an administrative unit; and a university hospital. The medical expert assessment was carried out by general practitioners and psychiatrists who managed sick leave and return to work cases. Around 52% had up to three sick leave episodes, and 10% of the workers had 20 or more episodes. Each spell of sickness absence lasted a median of 30 days (IQR 8–60 days). Among all of sick leaves, 35% had as a primary cause mental or behavioural diseases, of which 30% were depressive disorders, followed by around 18% related to the musculoskeletal system and the connective tissues. In the medical reports, 80% of the workers reported pain and 30% reported psychological symptoms. The collective, seen as privileged by many for their job stability, has a high percentage of sick leave due to mental illness, with extended periods which affect the levels of disability and reduce possibilities of return.

          How to measure sickness absence? Literature review and suggestion of five basic measures.

          To examine different sick-leave measures used in sickness absence research, and to suggest a systematic way of assessing sickness absence. A review and analysis of five major studies on sick-leave performed 1983-1988 with an epidemiological approach. Terminology and measures used varied in the different studies reviewed. The choice of a certain measure was seldom discussed in relation to the aim of the study. Based on the review five measures are suggested: frequency, length, incidence rate, cumulative incidence and duration. The definition of incidence rate is new and is a measure useful in studies of recurrent events within epidemiology. We have reviewed sick-leave measures previously used in the literature and suggested five basic measures for assessing sick-leave.

              Condições de trabalho e transtornos mentais comuns em trabalhadores da rede básica de saúde de Botucatu (SP)

              Os transtornos mentais comuns (TMC) apresentam elevada prevalência em populações gerais e de trabalhadores, com consequências individuais e sociais importantes. Este estudo, transversal e descritivo, explora a relação entre demandas psicológicas, grau de controle e presença de suporte social no trabalho e prevalência de TMC em trabalhadores da rede básica de saúde de Botucatu (SP). A coleta de dados foi feita por meio de questionário autoaplicável, não identificado, com destaque para itens relativos à demanda-controle-suporte e presença de TMC (Self Reporting Questionnaire, SRQ-20). As informações foram inseridas em banco de dados construído com Excel/Office XP 2003 e a análise estatística, efetuada com o programa SAS. Constatou-se que 42,6% dos trabalhadores apresentavam TMC. A observação de associação - alta prevalência de TMC com elevado desgaste (classificação de Karasek) e baixa prevalência de TMC com baixo desgaste - indica que, no município estudado, as condições de trabalho na atenção básica constituem fator contributivo não negligenciável ao adoecimento dos trabalhadores. Revela-se a necessidade de intervenções direcionadas ao cuidado aos trabalhadores, melhoria das condições de trabalho e aumento do suporte social no trabalho.
