          Abstract

          Resumen Marco contextual: En tiempos de crisis humanitarias, como en el caso de la pandemia del VIH, los adolescentes y jóvenes, quienes son poblaciones críticas en esta lucha, enfrentarán un futuro incierto y desolador si no se toman acciones a tiempo. Objetivo: Reflexionar sobre cómo las crisis humanitarias repercuten en la salud de adolescentes y jóvenes que viven con VIH y SIDA. Principales temas en análisis: Las crisis humanitarias influyeron en el acceso al diagnóstico de VIH, así como al tratamiento de los adolescentes y jóvenes que viven con el VIH y SIDA. Y las respuestas en el contexto de emergencias humanitarias están dirigidas más a la disponibilidad de suministros para abordar problemas como el hambre, la pobreza, la escasez de agua, la educación, la vivienda y la gestión de los ecosistemas. Conclusión: Las crisis humanitarias repercuten en la salud de los adolescentes y jóvenes que viven con VIH y SIDA y representan un gran desafío, ya que dificultan el acceso y la atención a los servicios de salud.

          Abstract Background: In the context of a humanitarian crisis such as the HIV pandemic, adolescents and young people, who are a critical population in this struggle, will face an uncertain and bleak future if action is not taken in time. Objective: To reflect on how humanitarian crises impact the health of adolescents and young people with HIV/AIDS. Main topics under analysis: Humanitarian crises impacted access to HIV diagnosis and treatment for adolescents and young people with HIV/AIDS. In humanitarian emergencies, responses focus on providing supplies to address issues like hunger, poverty, water scarcity, lack of education and shelter, and ecosystem management. Conclusion: Humanitarian crises have an impact on the health of adolescents and young people with HIV/AIDS and pose a major challenge in terms of access to and care for health services.

          Resumo Enquadramento: Em tempos de crises humanitárias, como no caso da pandemia do VIH, os adolescentes e os jovens, que são populações críticas nesta luta, enfrentarão um futuro incerto e sombrio se não forem tomadas medidas a tempo. Objetivo: Refletir sobre como as crises humanitárias repercutem na saúde de adolescentes e jovens que vivem com HIV e AIDS. Principais tópicos em análise: As crises humanitárias influenciaram o acesso ao diagnóstico do HIV, bem como ao tratamento de adolescentes e jovens que vivem com HIV e AIDS. E as respostas no contexto das emergências humanitárias centram-se mais na disponibilidade de suprimentos para evitar a fome, a pobreza, a água, a educação, a habitação e a gestão dos ecossistemas. Conclusão: As crises humanitárias repercutem na saúde de adolescentes e jovens que vivem com HIV e representam um grande desafio, pois dificultam o acesso e o atendimento aos serviços de saúde.

          The role of context in shaping HIV testing and prevention engagement among urban refugee and displaced adolescents and youth in Kampala, Uganda: findings from a qualitative study

          Abstract Objective To explore experiences, preferences and engagement with HIV testing and prevention among urban refugee and displaced adolescents and youth in Kampala, Uganda, with a focus on the role of contextual factors in shaping access and uptake. Methods This qualitative community‐based study with urban refugee and displaced youth aged 16–24 living in Kampala’s informal settlements involved five focus groups (FG), including two with young women, two with young men, and one with sex workers from March to May 2019. We also conducted five in‐depth key informant interviews. We conducted thematic analysis informed by Campbell and Cornish’s conceptualisation of material and symbolic contexts. Results Refugee/displaced youth participants (n = 44; mean age: 20.25, SD: 2.19; men: n = 17; women: n = 27) were from the Democratic Republic of Congo (n = 29), Rwanda (n = 11), Burundi (n = 3) and Sudan (n = 1). Participant narratives reflected material and symbolic contexts that shaped HIV testing awareness, preferences and uptake. Material contextual factors that presented barriers to HIV testing and prevention engagement included transportation costs to clinics, overcrowded living conditions that limited access to private spaces, low literacy and language barriers. Symbolic contexts that constrained HIV testing engagement included medical mistrust of HIV testing and inequitable gender norms. Religion emerged as an opportunity to connect with refugee communities and to address conservative religious positions on HIV and sexual health. Conclusion Efforts to increase access and uptake along the HIV testing and prevention cascade can meaningfully engage urban refugee and displaced youth to develop culturally and contextually relevant services to optimise HIV and sexual health outcomes.

            Impact of Ukrainian- Russian War on Health Care and Humanitarian Crisis.

            Authors explore the influence of Russian aggression on Ukrainian health care and humanitarian crises. On February 24, 2022, Ukraine faced an unprovoked brutal Russian invasion resulting in multiple negative consequences. During the 3 mo of full-scale war, Russians damaged more than 600 hospitals and killed at least 12 medics. Access to health care is severely impacted due to security concerns, restricted mobility, broken supply chains, and mass population displacement. Health care continues to come under attack (including attacks against health facilities, transport, personnel, patients, supplies, and warehouses), with a total of 295 attacks on health care, resulting in 59 injuries and 76 deaths, reported between February 24 and June 15. Ministry of Health reported that 1658 medical workers had been forced to change their residence and became employed in other regions of the country. Russian aggression negatively influenced not only the medical system and economics, but it led to a humanitarian crisis in the whole of Ukraine.

              From surviving to thriving: integrating mental health care into HIV, community, and family services for adolescents living with HIV

              Adolescents are a crucial generation, with the potential to bring future social and economic success for themselves and their countries. More than 90% of adolescents living with HIV reside in sub-Saharan Africa, where their mental health is set against a background of poverty, familial stress, service gaps, and an HIV epidemic that is now intertwined with the COVID-19 pandemic. In this Series paper, we review systematic reviews, randomised trials, and cohort studies of adolescents living with and affected by HIV. We provide a detailed overview of mental health provision and collate evidence for future approaches. We find that the mental health burden for adolescents living with HIV is high, contributing to low quality of life and challenges with adherence to antiretroviral therapy. Mental health provision is scarce, infrastructure and skilled providers are missing, and leadership is needed. Evidence of effective interventions is emerging, including specific provisions for mental health (eg, cognitive behavioural therapy, problem-solving, mindfulness, and parenting programmes) and broader provisions to prevent drivers of poor mental health (eg, social protection and violence prevention). We provide evidence of longitudinal associations between unconditional government grants and improved mental health. Combinations of economic and social interventions (known as cash plus care) could increase mental health benefits. Scalable delivery models include task sharing, primary care integration, strengthening families, and a pyramid of provision that differentiates between levels of need, from prevention to the care of severe disorders. A turning point has now been reached, from which complacency cannot persist. We conclude that there is substantial need, available frameworks, and a growing evidence base for action while infrastructure and skill acquisition is built.
