          Abstract

          Cardiac myxoma is the most common primary cardiac tumor. Rarely it can present with systemic or coronary embolization due to fragmentation of the tumor mass. We present a case of a young male who presented with an acute myocardial ischemia secondary to embolization of a left atrial myxoma originating from the left atrium. The patient underwent successful emergency surgical management of both the myxoma and the occlusion of the coronary artery. In this scenario, the surgery is the only effective treatment. The case also highlights the significance of performing emergency echocardiography in the setting of acute myocardial ischemia to look for possible associated pathology which can inform management plan.

          Acute myocardial infarction caused by a septic coronary embolism diagnosed and treated with a thrombectomy catheter.

          Acute myocardial infarctions are common in bacteraemia but are seldom diagnosed during life. A 64 year old man with severe chest pain who had fever for several days due to possible bacteraemia was shown by ECG and echocardiography to have possible lateral infarction. Immediate coronary angiography showed possible thrombus in the left circumflex artery, which was treated by thrombectomy catheter. Bacterial thrombus was removed and was verified by histological examination. A stent was implanted without complications. Acute myocardial infarction caused by septic embolism is usually fatal; however, thrombectomy may be useful in these cases.

            Coronary embolism complicating aortic valve endocarditis: treatment with placement of an intracoronary stent.

            A 37-year-old man, who had received 3 weeks of antimicrobial therapy for aortic value endocarditis, presented with an acute anteroseptal wall myocardial infarction. Coronary angiography demonstrated occlusion of the mid left anterior descending artery, thought to have been caused by embolization of a sterile vegetation. Following failure of balloon dilation to achieve vessel patency, this was achieved by placement of an intracoronary stent.

              Left main coronary artery thrombus: a case series with different outcomes.

              Left main coronary artery (LMCA) thrombosis with acute myocardial infarction is an uncommon condition with an extremely high mortality. The small number of reported cases prevents the development of an evidence-based approach. Hence there are no clear-cut guidelines describing the best management approach for this condition. We describe our experience with six patients who presented with LMCA thrombosis and discuss the epidemiology, etiology and management options available for this high-risk subgroup.
