          Abstract

          Les effets d’une perfusion de phosphates de sodium sur I’excrétion rénale des ions H<sup>+</sup> ont été étudiés chez dix malades de sexe féminin lors de la réapparition de I’excrétion urinaire au cours de néphropathies tubulo-interstitielles d’etiologies diverses. La perfusion phosphatée élève I’acidité titrable à des taux qui, ramenés à la filtration glomérulaire, sont identiques à ceux du rein normal. L’élévation de I’acidité titrable dépend de la quantité de tampons éliminés. Le fait est noté en I’absence d’acidose métabolique et malgré une diminution considérable de la filtration glomérulaire. Le pH urinaire reste fixe et anormalement haut durant I’épreuve. L’élévation de I’acidité ne s’accompagne pas d’une modification du rapport acidité titrable sur acidité titrable théorique maxima, même en cas d’acidose métabolique importante. La physiopathologie des faits rapportés, leurs liens avec les conditions notées spontanément lors de I’élévation du volume urinaire des néphropathies tubulo-interstitielles aiguës sont discutés. II est possible que dans de telles circonstances I’augmentation des accepteurs de protons émis par néphron masque un éventuel deficit tubulaire. De toute façon, celui-ci est moins intense qu’on aurait pu le penser.
