          Abstract

          Este estudio tuvo como objetivo probar un novedoso método de medición del cumplimiento en farmacias comunitarias portuguesas con pacientes con Helicobacter pylori. Se utilizó un diseño de serie de casos en los que se medían electrónicamente los indicadores del cumplimiento, a la vez que los informes de los pacientes. También se midieron las reacciones adversas medicamentosas, los beneficios del tratamiento percibidos y la calidad de vida. La proporción media de cumplimiento fue del 56% y se encontró un impacto positivo en el estado de salud percibido e los pacientes. El método utilizado fue bien acogido por los farmacéuticos comunitarios, además de que tuvo una influencia en la actuación normal de los pacientes.

          Translated abstract

          This study aimed to test a novel method of compliance measurement in Portuguese community pharmacy in Helicobacter pylori patients. A case series design was used where compliance indicators were electronically measured, aside with patients’ reports. Experienced adverse drug reactions, perceived benefit of therapy and quality of life were also measured. Mean compliance proportion was 56% and a positive impact on patients’ perceived health status was found. The method used was welcomed by community pharmacists, albeit having an influence on patients’ normal behaviour.

          Este estudo tinha por objectivo testar um método inovador para medir a adesão à terapêutica em doentes infectados com H. pylori na farmácia comunitária Portuguesa. Um estudo de casos foi utilizado, no qual os indicadores de adesão foram obtidos através de dois métodos: método electrónico e relato dos doentes. Estes referiram-se à forma como utilizaram os medicamentos, bem como a reacções adversas sentidas, benefício da terapêutica e estado de saúde percepcionado. A proporção média de adesão encontrada foi de 56%. Detectou-se um impacto positivo no estado de saúde percepcionado. O método utilizado foi considerado exequível pelos farmacêuticos comunitários, apesar de poder ter tido influência no comportamento normal dos doentes sob observação.

            Methods for measuring and monitoring medication regimen adherence in clinical trials and clinical practice.

            Researchers and clinicians have used numerous methods in their attempts to adequately assess patient compliance (adherence) with medication regimens and to identify noncompliant patients. Large variations have been reported in the extent of noncompliance in individual patients and large populations. In addition, nonadherence has often been poorly defined. Direct measures of adherence include drug assays of blood or urine, use of drug markers with the target medication, and direct observation of the patient receiving the medication. Indirect measures of adherence imply that the medication has been used by the patient; these measures include various forms of self-reporting by the patient, medication measurement (pill count), use of electronic monitoring devices, and review of prescription records and claims. Compliance measures should be assessed on the basis of their validity (sensitivity and specificity or statistical correlation) and the reference standard used. Many early studies used pill counts as a reference standard, but electronic monitoring devices such as the Medication Event Monitoring System have replaced pill counts as the reference standard. The choice of a method for measuring adherence to a medication regimen should be based on the usefulness and reliability of the method in light of the researcher's or clinician's goals. Specific methods may be more applicable to certain situations, depending on the type of adherence being assessed, the precision required, and the intended application of the results.
