          Abstract

          Resumo Introdução No Brasil, vislumbraram-se avanços significativos nas políticas de alimentação e nutrição. Apesar disso, o país apresenta milhões de pessoas em condições de Insegurança Alimentar (IA), com reconhecida dificuldade de acesso à alimentação adequada. Objetivo Esta pesquisa teve como objetivo investigar a associação entre insegurança alimentar com o perfil antropométrico e socioeconômico de agricultores dos municípios da região centro-sul do Ceará. Método Trata-se de uma pesquisa transversal que utilizou um questionário socioeconômico e a Escala Brasileira de Insegurança Alimentar (EBIA) para avaliar a situação de IA e o Índice de Massa Corporal (IMC) para avaliação do estado nutricional. Os testes de Mann-Whitney e qui-quadrado foram utilizados, considerando significativo p < 0,05. Resultados Participaram desta pesquisa 86 agricultores, destes 39,5% em condições de IA e 41% com sobrepeso. Foi identificada associação entre a situação de IA em famílias com o maior número de indivíduos (p < 0,05), a presença de menores de 18 anos (p < 0,05) e menor renda per capita (p = 0,001). Conclusão Podemos concluir que a IA esteve associada fortemente à aglomeração domiciliar e renda entre famílias de agricultores. Assim, as políticas públicas voltadas à saúde dos agricultores devem considerar a vulnerabilidade das famílias de baixa renda e com crianças e jovens na sua composição, possibilitando acesso à alimentação adequada.

          Translated abstract

          Abstract Background In Brazil, it has been envisioned significant advances in food and nutrition policies. Despite this, the country has millions of people under conditions of Food Insecurity (FI), with recognized difficulties of access to adequate food. Objective This research aimed to investigate the association between FI with anthropometric and socioeconomic statusof farmers of the municipalities in the Middle-South region of Ceará. Method It is a cross-sectional study that used a socioeconomic questionnaire and the Brazilian Food Insecurity Scale to assess the situation of FI and the Body Mass Index (BMI) for assessing the nutritional status. The tests of Mann-Whitney and Chi-square were used, considering a significant p-value (p<0.05). Results 86 farmers participated in this research, including 39.5% of those who wereFI and 41% overweight. The association was identified between the situation of FI in families with the greatest number of individuals (p<0.05), the presence of individuals aged under18 years (p<0.05) and lower per capita income (p=0.001). Conclusion It could be conclude that the FI was strongly associated to household crowding and income among farmers’ families Thus, the public policies directed to the farmers’ health should consider the vulnerability of low-income families and families with children as well as young people in their composition, enabling access to adequate food.

          Screening for nutritional status in the elderly.

          A comprehensive assessment of nutritional status is a critically important component of any patient evaluation. Based upon clinical information, anthropometric data, and a small number of laboratory investigations, an accurate appraisal of nutritional status should be possible and an appropriate intervention plan can be developed. The actual approach depends on the particular problem discovered. These are discussed in detail elsewhere in this issue.

            Food insecurity measurement and indicators

            The United Nations define food security as "People having at all times, physical, social and economic access to sufficient, safe and nutritious food which meets their dietary needs and food preferences for an active and healthy life." There are five methods that are commonly applied in national surveys that can be used to assess food insecurity. Of these, four are indirect or derivative measures of food insecurity (United Nations Food and Agriculture Organization method, household expenditure surveys, dietary intake assessment and anthropometry). The only method that represents a fundamental or direct measure of food insecurity is the one based on experience-based food insecurity scales. All the methods complement each other and the method of choice depends on the question being answered and the economic and logistical resources available to collect valid data. All the methods have serious measurement error issues that can be reduced by fully understanding the principles underlying them and the use of highly trained and standardized research field workers. As shown in Brazil, the use of experience-based food insecurity measurement scales for mapping, targeting, and understanding the determinants and consequences of food insecurity is very promising. Thus, we recommend the Latin American and Caribbean Region to work towards the adoption of a single regional module that can be adapted to the local contexts based on qualitative cognitive research followed by quantitative confirmation of the scale's psychometric properties. The Brazilian experience-based food insecurity measurement project is likely to provide useful insights to other countries in the region.

              Insegurança alimentar como indicador de iniqüidade: análise de inquérito populacional

              O objetivo deste trabalho foi investigar a associação da insegurança alimentar com algumas variáveis indicativas de desigualdades sociais, como renda, escolaridade, raça/cor, composição familiar, características da moradia e condições de saneamento. Para medida de segurança alimentar, aplicou-se o instrumento EBIA (Escala Brasileira de Insegurança Alimentar) a uma amostra de 456 famílias residentes em área urbana do Município de Campinas. Verificou-se que a insegurança alimentar concentrou-se em famílias com maior número de membros menores de 18 anos, vivendo em construções precárias e com alta aglomeração de moradores, sem rede de esgoto, de baixa renda (menos de dois salários mínimos), cujo responsável não freqüentou a escola, sem membros com nível universitário e nas quais os informantes referiram ter cor da pele preta. Considera-se que a medida direta de segurança alimentar por meio da EBIA seja um importante indicador para monitoramento da iniqüidade, podendo complementar um conjunto de indicadores sociais ou, mesmo de forma isolada, identificar grupos com vulnerabilidade social.
