O tema da apresentação de pacientes, dispositivo clínico utilizado tanto pela psiquiatria quanto pela psicanálise é um importante ponto de partida para a discussão sobre as diferenças epistêmicas existentes entre estas duas áreas. Freud presenciou as famosas apresentações de pacientes protagonizadas por Charcot na Salpêtrière, momento imediatamente anterior à criação da psicanálise. Enquanto psicanalista, ele não praticou as apresentações de pacientes, no entanto, seus relatos de caso, entre eles o que ficou conhecido como "O homem dos ratos", funcionam como porta-voz do sujeito do inconsciente, mantendo viva a ruptura que produziu no discurso da ciência.
