      Tratamiento con vasodilatadores de la falla de bomba en el infarto agudo de miocardio

          Abstract

          Se emprendió este estudio para valorar los efectos que el dinitrato de isosorbide, el nitroprusiato sódico y el prazosin tuvieron en el aparato cardiocirculatorio de un grupo de pacientes con falla de bomba por infarto agudo del miocardio (IAM), integrado por 39 sujetos, de las cuales 25 eran varones y 14 mujeres con edades que oscilaban entre los 37 y los 69 años; todos ellos cumplian los criterios establecidos por las unidades de cuidado coronario intensivo, en cuanto al diagnóstico de IAM y falla de bomba.Los cambios observados en la frecuencia cardiaca y en la tension arterial media no fueron significativos para ninguno de los tres fármacos. La presión capilar se redujo en forma significativa y en un grado similar con las tres drogas. En cambio, tanto el aumento del Indice Cardiaco, Volumen Sistólico e Indice de Trabajo Sistólico de Ventrículo Izquierdo, así como la disminución de la resistencia periférica total, solo resultaron significativos para el nitroprusiato y el prazosin, pues si bien el isosorbide mostró una tendencia semejante, los valores alcanzados carecieron de significación.
