          Abstract

          O paradigma do "uso seguro" de agrotóxicos sustenta-se em medidas de controle dos riscos na manipulação desses produtos. No entanto, estudos realizados em diversas regiões do País revelam um quadro de exposição e danos à saúde de trabalhadores rurais, evidenciando a ineficácia deste paradigma. Este trabalho apresenta uma revisão crítica sobre a abordagem do "uso seguro" de agrotóxicos nos artigos científicos publicados nos últimos 15 anos no Brasil. Os resultados apontaram que esses estudos não abordam, simultaneamente, todas as atividades de trabalho que envolvem exposição e risco de intoxicação (aquisição, transporte, armazenamento, preparo e aplicação, destino final de embalagens vazias e lavagem de roupas/EPI contaminados), tampouco abordam de maneira abrangente as diversas medidas de "uso seguro", descritas pelos manuais de segurança, obrigatórias para cada atividade. No total, 25 artigos foram selecionados e analisados, evidenciando-se a concentração de resultados e análises nas atividades de preparo e aplicação e destino final de embalagens vazias. A abrangência das abordagens se mostrou pontual nas seis atividades. Para futuros trabalhos, sugere-se uma abordagem ampliada do "uso seguro" de agrotóxicos, buscando-se revelar a completa inviabilidade deste paradigma de segurança.

          Translated abstract

          The paradigm of the "safe use" of pesticides is based on measures to control risks in the handling of these products. However, studies carried out in various regions of Brazil reveal a situation of widespread exposure and health damages among rural workers, revealing the ineffectiveness of this paradigm. This work presents a critical review of the "safe use" approach for pesticides in scientific papers published in Brazil in the past 15 years. Results indicate that these studies do not address, simultaneously, all the work activities that involve exposure and risk of intoxication (acquisition, transportation, storage, preparation and application, final disposal of empty containers and sanitization of contaminated clothes/ PPEs), nor do they comprehensively address the "safe use" measures recommended in safety manuals, which are mandatory for each activity. A total of 25 studies were selected and analyzed, revealing a high number of results and analyses regarding activities of preparation and application and final disposal of empty containers. The range of the approaches was seen to be timely in the six work activities. For future studies, a broader approach of the "safe use" of pesticides is recommended, seeking to reveal the complete infeasibility of this safety paradigm.

            Neuropsychiatric evaluation in subjects chronically exposed to organophosphate pesticides.

            Long-term exposure to low levels of organophosphate pesticides (OP) may produce neuropsychiatric symptoms. We performed clinical, neuropsychiatric, and laboratory evaluations of 37 workers involved in family agriculture of tobacco from southern Brazil who had been exposed to OP for 3 months, and in 25 of these workers, after 3 months without exposure to OP. Plasma acetylcholinesterase activity levels of all subjects were within the normal range (3.2 to 9.0 U/l) and were not different between on- and off-exposure periods (4.7 +/- 0.9 and 4.5 +/- 1.1 U/l, respectively). Clinically significant extrapyramidal symptoms were present in 12 of 25 subjects, which is unexpected in such a population. There was a significant reduction of extrapyramidal symptoms after 3 months without exposure to OP, but 10 subjects still had significant parkinsonism. Mini-mental and word span scores were within the expected range for this population and were not influenced by exposure to OP. Eighteen of the 37 subjects (48%) had current psychiatric diagnoses in the first interview (13 with generalized anxiety disorder and 8 with major depression). Among the 25 subjects who completed both evaluations, the total number of current psychiatric diagnoses, after 3 months without using OP, dropped from 24 to 13 and the number of affected individuals with any psychiatric diagnosis dropped from 11 to 7. In conclusion, this study reinforces the need for parameters other than acetylcholinesterase activity to monitor for chronic consequences of chronic low-dose OP exposure, and it suggests that subjects have not only transient motor and psychiatric consequences while exposed, but may also develop enduring extrapyramidal symptoms.

              Acidente rural ampliado: o caso das "chuvas" de agrotóxicos sobre a cidade de Lucas do Rio Verde - MT

              O artigo aborda o acidente ambiental causado por derivas de pulverizações aéreas de agrotóxico que atingiram o espaço urbano de Lucas do Rio Verde-MT, em março de 2006. Caracterizou-se como "acidente rural ampliado" de caráter ocupacional e ambiental, cuja gravidade e extensão ultrapassaram a unidade produtiva rural, causando impactos sanitários, sociais e ambientais. Este estudo de caso objetivou conhecer o cenário sociotécnico do acidente e o processo de vigilância em saúde-ambiente, numa dinâmica de pesquisa-ação. As informações foram colhidas através de entrevistas, documentos e registro de observações do cotidiano. Referenciou-se ainda na análise interdisciplinar e participativa de acidentes, com envolvimento de instituições locais de Saúde, Agricultura e Ambiente, lideranças sindicais e políticas, chacareiros e fazendeiros, Promotoria de Justiça, jornalistas e a Universidade. O estudo mostra que as ações de vigilância do "uso e abuso" de agrotóxicos ampliaram-se para "movimento pelo desenvolvimento sustentável da região", apoiado na vigilância participativa, articulada com a luta pela democracia e justiça social, na busca de uma agricultura e/ou ambiente sustentável.
