          Abstract

          RESUMEN Objetivo Identificar las publicaciones científicas sobre eventos adversos relacionados con las prácticas asistenciales y discutir la cultura de seguridad del paciente. Métodos Se realizó una revisión bibliográfica del tipo integrador, con búsqueda en MEDLINE, LILACS y BDENF disponibles en la BVS, utilizando los siguientes descriptores; seguridad del paciente, asistencia al paciente, vigilancia de eventos centinela, gestión de la seguridad. Cumplieron los criterios de inclusión 26 artículos científicos publicados en el período delimitado para esta revisión de 2005 a 2015. Resultados El análisis de los artículos reveló cinco categorías principales: características de los eventos adversos relacionados con las prácticas asistenciales, implicaciones de la ocurrencia de eventos adversos, medidas para la prevención de eventos adversos, notificaciones de los eventos adversos y los factores intervenientes y el conocimiento y la cultura de la seguridad. Conclusión La cultura punitiva aparece como responsable de la subnotificación y omisión de los eventos adversos, que constituyen un obstáculo para la aplicación efectiva de las investigaciones. Es esencial estimular una cultura de seguridad para establecer estrategias que aseguren la prestación de cuidados libres de daños.

          Translated abstract

          ABSTRACT Objective To identify the scientific publications about adverse events related to practical assistance and discuss the culture of patient safety. Methods An integrative literature review was done through research in MEDLINE, LILACS and BDENF available in BVS, using the following descriptors: patient safety, patient assistance, surveillance of sentinel event, safety management. 26 scientific articles published in the period defined for this review, 2005 to 2015 met the inclusion criteria. Results The analysis of the articles revealed five major categories: characteristics of adverse events related to practical assistance, adverse events occurrence implications, measures for the prevention of adverse events, notifications of adverse events and the intervenient factors and the knowledge and the culture of safety. Conclusion The punitive culture comes as responsible for underreporting and omission of adverse events, being constituted a barrier to effective implementation of investigations. It is essential to foster a safety culture to establish strategies that ensure the care provision free of damage.

Resultados A análise dos artigos revelou cinco categorias importantes: características dos eventos adversos relacionados às práticas assistências, implicações da ocorrência de eventos adversos, medidas de prevenção de eventos adversos, notificações de eventos adversos e os fatores intervenientes e o conhecimento e a cultura de segurança. Conclusão A cultura punitiva aparece como responsável pela subnotificação e omissão dos eventos adversos, constituindo-se uma barreira para realização de investigações eficazes. É imperativo estimular a cultura de segurança para que se estabeleçam estratégias que garantam a prestação de cuidados livres de danos.

            The Critical Care Safety Study: The incidence and nature of adverse events and serious medical errors in intensive care.

            Critically ill patients require high-intensity care and may be at especially high risk of iatrogenic injury because they are severely ill. We sought to study the incidence and nature of adverse events and serious errors in the critical care setting. We conducted a prospective 1-year observational study. Incidents were collected with use of a multifaceted approach including direct continuous observation. Two physicians independently assessed incident type, severity, and preventability as well as systems-related and individual performance failures. Academic, tertiary-care urban hospital. Medical intensive care unit and coronary care unit patients. None. The primary outcomes of interest were the incidence and rates of adverse events and serious errors per 1000 patient-days. A total of 391 patients with 420 unit admissions were studied during 1490 patient-days. We found 120 adverse events in 79 patients (20.2%), including 66 (55%) nonpreventable and 54 (45%) preventable adverse events as well as 223 serious errors. The rates per 1000 patient-days for all adverse events, preventable adverse events, and serious errors were 80.5, 36.2, and 149.7, respectively. Among adverse events, 13% (16/120) were life-threatening or fatal; and among serious errors, 11% (24/223) were potentially life-threatening. Most serious medical errors occurred during the ordering or execution of treatments, especially medications (61%; 170/277). Performance level failures were most commonly slips and lapses (53%; 148/277), rather than rule-based or knowledge-based mistakes. Adverse events and serious errors involving critically ill patients were common and often potentially life-threatening. Although many types of errors were identified, failure to carry out intended treatment correctly was the leading category.

              Sobrecarga de trabalho da Enfermagem e incidentes e eventos adversos em pacientes internados em UTI

              Estudo prospectivo, tipo coorte que visou identificar a influência da sobrecarga de trabalho da Enfermagem na ocorrência de incidentes sem lesão e eventos adversos em 399 pacientes internados em Unidades de Terapia Intensiva (UTI). Para coleta de dados, foi aplicado um questionário estruturado e feita análise de prontuários. Nessas admissões, aproximadamente 78% dos incidentes sem lesão e eventos adversos em pacientes foram relacionados à esfera da Enfermagem. Essas ocorrências foram atribuídas à sobrecarga de trabalho, aumentaram o número de dias de internação e o risco de óbito dos pacientes estudados. É fundamental que os gerentes de enfermagem atuem no processo de gestão de pessoas no âmbito hospitalar, evitando a sobrecarga de trabalho e proporcionando, consequentemente, aumento da segurança do paciente.
