      Abstract

      Durante el período comprendido entre Diciembre de 1999 y Abril del año 2000, se realizó un análisis descriptivo-exploratorio y un análisis factorial de correspondencias múltiples, con una muestra de 129 mujeres atendidas en la Unidad de Cáncer de Cervix de la Universidad de Caldas y en las veredas el Tablazo, el Kilómetro 41 y la Cabaña, con el fin de determinar si hay o no hay relación entre las conductas sexuales y algunas patologías crónicas del tracto genital inferior femenino. La recolección de la información se hizo mediante la utilización de un formulario que contemplaba aspectos socio-demográficos, clínicos, de conductas sexuales (información primaria) y datos de la historia clínica (información secundaria). La información obtenida se sometió a un análisis factorial de correspondencias múltiples y a un análisis de Clusters, éstos se llevaron a cabo con base en el software SPAD-WIN. Los resultados mostraron que, no existe relación directa entre el VPH y la edad de inicio de las relaciones sexuales. Además la relación entre ITS, especialmente el VPH, y el número de compañeros sexuales no está claramente establecida. Finalmente la falta de protección es considerada una conducta de altísimo riesgo para la infección por VPH y otras ITS.

      Translated abstract

      During the time period between December 1999 and April 2000 an exploratory descriptive analysis and a factorial analysis of multiple correspondences were carried out in a sample of 129 women, who were admitted in the Cervix Cancer Unit of Universidad de Caldas and in the municipal rural settlements of El Tablazo, La Cabaña, El KM 41, with the purpose of determining the existence of the relation between sexual behaviors and some chronic pathologies of the lower female genital tract. The collection of the information was carried out by means of the use of a survey that included sociodemographic, clinical and sexual behavioral aspects (primary information) and data from the clinical histories (secondary information). The information obtained was submitted to a factorial analysis of multiple correspondences and to a cluster analysis and they were carried out using the SPAD-WIN software. The partial results obtained allowed us to deduce that there is not a direct relation between the Human Papillomaviruses HPV and the initiation age of sexual activity, moreover the relation between STDs, specially HPV, and the number of sexual partners is not clearly established. Finally, the lack of protection is considered as a high risk behavior for acquiring HPV and other STDs.

      Durante o período compreendido entre dezembro de 1999 e abril di ano 2000 realizou se um analise descritivo exploratório e um analise fatorial de correspondências múltiplas com uma mostra de 129 mulheres atendidas na Unidade de Câncer de Cerviz da Universidade de Caldas e nas Veredas o Tablazo, o Quilômetro 41 e a Cabana; com o fim de determinar a existência o não da relação entre as condutas sexuais e algumas patologias crônicas do conduto genital inferior feminino. A relação da informação se fez mediante a utilização dum formulário que contemplava aspectos sócio-demograficos, clínicos, de condutas sexuais (informação primaria) e apoio da historia clinica (informação secundaria). La informação obtida submeteu se a um analise fatorial de correspondências múltiplas e a um analisem de Clusters, estes levaram se a cabo com base no software SPAD-WIN. Os resultados demonstram que não existe ralação direita entre o VPH, e a idade de inicio das relações sexuais. Além a relação entre ITS, especialmente El VPH, e o numero de companheiros sexuais não esta claramente estabelecida e finalmente a falta de proteção é considerada uma conduta de altíssimo risco para a infecção por VPH e outras ITS.

      The epidemiology of cervical carcinogenesis.

      Epidemiologic and laboratory data suggest that cervical cancer typically arises from a series of causal steps. Each step can be studied separately in the hope of better etiologic understanding and improved cancer prevention. The earliest identified etiologic step is infection of young women with specific types of venereally transmissible human papillomaviruses (HPVs). Cervical HPV infections often lead to low grade squamous intraepithelial lesions (mildly abnormal Pap smears). Human papillomavirus infections and their associated lesions are extremely common among young, sexually active women. The infections typically resolve spontaneously even at the molecular level within months to a few years. Uncommonly, HPV infections and/or low grade lesions persist and progress to high grade lesions. The risk factors for progression are mainly unknown but include HPV type and intensity, cell-mediated immunity, and reproductive factors. Nutritional factors or co-infection with other pathogens may also be involved at this apparently critical etiologic step between common low grade and uncommon high grade intraepithelial lesions. Except for advancing age, no epidemiologic risk factors have been found for the next step between high grade intraepithelial lesions and invasive cancer. At the molecular level, invasion is associated with integration of viral DNA. Based on worldwide research, the steps in cervical carcinogenesis appear to be fundamentally the same everywhere, with a central role for HPV infection. The importance of etiologic cofactors like smoking, however, may vary by region.
