          Abstract

          Le cancer du sein chez l'homme est une affection rare représentant environ 1% de tous les cancers du sein et moins de 1% de l'ensemble des néoplasies masculines. L'objectif de notre étude est de décrire les différents aspects cliniques, histologique, pronostic et thérapeutiques de cette tumeur rare afin de contribuer à l'amélioration de la prise en charge de cette affection. Il s'agit d'une étude rétrospective portant sur 40 patients colligés au Centre Mohamed VI pour le traitement des cancers de Casablanca de Janvier 2000 à Décembre 2012. L'âge médian a été de 62 ans, le délai moyen de consultation a été de 12 mois, l'autopalpation d'un nodule péri aréolaire a été le principal motif de consultation dans 90% des cas. Le carcinome canalaire infiltrant a été le type histologique prédominant dans 90% des cas. La prise en charge thérapeutique multimodale a consisté en une mastectomie suivie d'un traitement adjuvant à type de chimiothérapie, radiothérapie et/ou hormonothérapie en fonction du stade de la tumeur et des caractéristiques histologiques. Le suivi moyen a été de 38 mois, l'évolution a été caractérisée par une rémission complète chez 16 patients (soit 40%), rechute locale chez 3 patients (soit 7.5%) et par une rechute métastatique chez 5 patients (soit 12.5%). Le site métastatique le plus fréquent a été l'os (62%), suivi par le poumon et le foie. Le décès était survenu chez 10 patients (25%). Le cancer du sein chez l'homme présente des similitudes avec le cancer mammaire chez la femme. Mais il présente aussi beaucoup de particularités, d'où l'intérêt de mener des études prospectives randomisés à plus large échelle afin d'améliorer la prise en charge et le pronostic de cette affection dont l'impact psychosociale est considérable.

          Translated abstract

          Breast cancer in men is rare, accounting for approximately 1% of all breast cancers and less than 1% of all neoplasias in men. This study aimed to highlight the clinical histological, prognostic and therapeutic features of this rare tumor in order to contribute to improve the management of these patients. We conducted retrospective study of 40 patients whose data were collected at the Mohammed VI Center for Cancers Treatment in Casablanca from January 2000 to December 2012. The average age was 62 years, the mean consultation time was 12 months, self-exam of a peri-areolar nodul was the main reason for consultation in 90% of cases. Infiltrating ductal carcinoma was the predominant histological type in 90% of cases. Multimodal treatment was based on mastectomy followed by adjuvant therapy including chemotherapy, radiation therapy and/or hormonal therapy, depending on tumor stage and its histological features. The mean follow-up time was 38 months, patient's evolution was characterized by complete remission in 16 patients (40%), local recurrence in 3 patients (7.5%) and metastatic recurrence in 5 patients (12.5%). Metastases mainly occurred in the bones (62%), followed by the lungs and the liver. 10 patients (25%) died. Breast cancer in men is similar to breast cancer in women. However it has its own peculiarities, hence the importance of conducting broader prospective randomised studies in order to improve the treatment and the prognosis of this disease with a high psychosocial impact.

          Multidisciplinary meeting on male breast cancer: summary and research recommendations.

          Male breast cancer is a rare disease, accounting for less than 1% of all breast cancer diagnoses worldwide. Most data on male breast cancer comes from small single-institution studies, and because of the paucity of data, the optimal treatment for male breast cancer is not known. This article summarizes a multidisciplinary international meeting on male breast cancer, sponsored by the National Institutes of Health Office of Rare Diseases and the National Cancer Institute Divisions of Cancer Epidemiology and Genetics and Cancer Treatment and Diagnosis. The meeting included representatives from the fields of epidemiology, genetics, pathology and molecular biology, health services research, and clinical oncology and the advocacy community, with a comprehensive review of the data. Presentations focused on highlighting differences and similarities between breast cancer in males and females. To enhance our understanding of male breast cancer, international consortia are necessary. Therefore, the Breast International Group and North American Breast Cancer Group have joined efforts to develop an International Male Breast Cancer Program and to pool epidemiologic data, clinical information, and tumor specimens. This international collaboration will also facilitate the future planning of clinical trials that can address essential questions in the treatment of male breast cancer.

            Epidemiology of male breast cancer.

            Breast cancer in men is a rare disease, accounting for approximately 1% of all breast cancer cases. Although the epidemiologic literature regarding female breast cancer is extensive, relatively little is known about the etiology of male breast cancer (MBC). This review is intended to summarize the existing body of evidence on genetic and epidemiologic risk factors for breast cancer in men. Overall, the epidemiology of MBC presents similarities with the epidemiology of female breast cancer. Major genetic factors associated with an increased risk of breast cancer for men include BRCA2 mutations, which are believed to account for the majority of inherited breast cancer in men, Klinefelter syndrome, and a positive family history. Suspected genetic factors include AR gene mutations, CYP17 polymorphism, Cowden syndrome, and CHEK2. Epidemiologic risk factors for MBC include disorders relating to hormonal imbalances, such as obesity, testicular disorders (e.g., cryptorchidism, mumps orchitis, and orchiectomy), and radiation exposure. Suspected epidemiologic risk factors include prostate cancer,prostate cancer treatment, gynecomastia, occupational exposures (e.g., electromagnetic fields, polycyclic aromatic hydrocarbons, and high temperatures), dietary factors (e.g., meat intake and fruit and vegetable consumption), and alcohol intake.
