      New Insights into Sepsis Therapy Using Sepia Officinalis

          Abstract

          Background:

          Sepsis remains a major problem for both scientists and clinicians. Cecal ligation and puncture (CLP) is considered the gold standard for animal models of sepsis. The undesirable side effects of certain antibiotics have forced scientists to discover new, natural, and safe antimicrobial agents, such as cephalopods, which are known to display significant antimicrobial activity.

          Objectives:

          The present investigation aims to evaluate the in vitro and in vivo antibacterial and antiseptic efficacy of Sepia officinalis body tissue (SOBT) extract and S. officinalis polysaccharide (SOP) from its cuttlebone.

          Materials and Methods:

          Forty-eight rats were divided into 4 groups, and starting 2 hours after CLP, treatments were given for 2 days as follows: sham control rats treated orally with distilled water, septic rats treated orally distilled water, septic rats treated orally methanolic extract of SOBT (500 mg/kg b.wt) suspended in distilled water, and septic rats treated orally SOP extract (200 mg /kg b.wt) dissolved in distilled water. On the third day, half of the rats in each group were euthanized for blood collection. The other half were kept alive and used for the survival study.

          Results:

          The present study revealed that the SOBT and SOP extracts showed in vitro bactericidal activity against gram-positive and gram-negative bacteria. Furthermore, administration of SOBT and SOP increased the rats’ survival rates by 66.7% and 83.33%, respectively, as compared to the untreated CLP-septic rats. Treatment of the CLP-septic rats with SOBT and SOP significantly alleviated alterations in procalcitonin levels and in some hematological parameters induced by CLP.

          Conclusions:

          SOBT and SOP had profound antiseptic efficacy.

          Antimicrobial properties of chitosan and mode of action: a state of the art review.

          Owing to its high biodegradability, and nontoxicity and antimicrobial properties, chitosan is widely-used as an antimicrobial agent either alone or blended with other natural polymers. To broaden chitosan's antimicrobial applicability, comprehensive knowledge of its activity is necessary. The paper reviews the current trend of investigation on antimicrobial activities of chitosan and its mode of action. Chitosan-mediated inhibition is affected by several factors can be classified into four types as intrinsic, environmental, microorganism and physical state, according to their respective roles. In this review, different physical states are comparatively discussed. Mode of antimicrobial action is discussed in parts of the active compound (chitosan) and the target (microorganisms) collectively and independently in same complex. Finally, the general antimicrobial applications of chitosan and perspectives about future studies in this field are considered. Copyright © 2010 Elsevier B.V. All rights reserved.

            Animal models of sepsis: setting the stage.

            Sepsis is a state of disrupted inflammatory homeostasis that is often initiated by infection. The development and progression of sepsis is multi-factorial, and affects the cardiovascular, immunological and endocrine systems of the body. The complexity of sepsis makes the clinical study of sepsis and sepsis therapeutics difficult. Animal models have been developed in an effort to create reproducible systems for studying sepsis pathogenesis and preliminary testing of potential therapeutic agents. However, demonstrated benefit from a therapeutic agent in animal models has rarely been translated into success in human clinical trials. This review summarizes the common animal sepsis models and highlights how results of recent human clinical trials might affect their use.

              Clinical review: The liver in sepsis

              During sepsis, the liver plays a key role. It is implicated in the host response, participating in the clearance of the infectious agents/products. Sepsis also induces liver damage through hemodynamic alterations or through direct or indirect assault on the hepatocytes or through both. Accordingly, liver dysfunction induced by sepsis is recognized as one of the components that contribute to the severity of the disease. Nevertheless, the incidence of liver dysfunction remains imprecise, probably because current diagnostic tools are lacking, notably those that can detect the early liver insult. In this review, we discuss the epidemiology, diagnostic tools, and impact on outcome as well as the pathophysiological aspects, including the cellular events and clinical picture leading to liver dysfunction. Finally, therapeutic considerations with regard to the weakness of the pertinent specific approach are examined.
