      Adverse drug reactions as cause of hospital admissions: results from the Italian Group of Pharmacoepidemiology in the Elderly (GIFA).

          Abstract

          To determine the prevalence of adverse drug reaction (ADR)-related hospital admissions in an older population, to describe the most common clinical manifestations and drugs most frequently responsible for ADR-related hospital admissions, and to identify independent factors predictive of these ADRs. Multicenter pharmacoepidemiology survey conducted between 1988 and 1997. Eighty-one academic hospitals throughout Italy. Twenty-eight thousand four hundred eleven patients consecutively admitted to participating centers during the survey periods. For each suspected ADR at admission, a physician, who coded description, severity, and potentially responsible drugs, completed a questionnaire. Mean age +/- standard deviation of the patients was 70 +/- 16. One thousand seven hundred four ADRs were identified upon hospital admission. In 964 cases (3.4% of all admissions), ADRs were considered to be the cause of these hospital admissions. Of these, 187 ADRs were coded as severe. Gastrointestinal complaints (19%) represented the most common events, followed by metabolic and hemorrhagic complications (9%). The drugs most frequently responsible for these ADRs were diuretics, calcium channel blockers, nonsteroidal antiinflammatory drugs, and digoxin. Female sex (odds ratio (OR) = 1.30, 95% confidence interval (CI) = 1.10-1.54), alcohol use (OR = 1.39, 95% CI = 1.20-1.60), and number of drugs (OR = 1.24, 95% CI = 1.20-1.27 for each drug increase) were independent predictors of ADR-related hospital admissions. For severe ADRs, age (OR = 1.50, 95% CI = 1.01-2.23 for age 65-79 and OR = 1.53, 95% CI = 1.00-2.33 for age > or =80, respectively), comorbidity (OR = 1.12, 95% CI = 1.05-1.20 for each point in the Charlson Comorbidity Index), and number of drugs (OR = 1.18, 95% CI = 1.11-1.25 for each drug increase) were the only predisposing factors. The most important determinant of risk for ADR-related hospital admissions in older patients is number of drugs being taken. When considering only severe ADRs, risk is also related to age and frailty.

            Validity of self-reported diagnoses leading to hospitalization: a comparison of self-reports with hospital records in a prospective study of American adults.

            The authors compared interview reports with hospitalization records of participants in a nationally representative survey to determine the accuracy of self-reports of ischemic heart disease, stroke, gallbladder disease, ulcers, cataract, hip fracture, colon polyps, and cancers of the colon, breast, prostate, and lung. The study cohort consisted of 10,523 participants from the First National Health and Nutrition Examination Survey in 1971-1975 who were aged 25-74 years at the baseline examination and who completed a follow-up interview in 1982-1984. Self-reports of hospitalization for breast cancer were confirmed as accurate for 100% of cases where a hospital record was available. Self-report accuracy was also high for ischemic heart disease (84%), cataract (83%), and hip fracture (81%); it was moderate for lung cancer (78%), prostate cancer (75%), gallbladder disease (74%), colon cancer (71%), and stroke (67%); but it was low for ulcers (54%) and colon polyps (32%). Some of the self-reports of ulcers (20%), hip fracture (9%), ischemic heart disease (7%), and stroke (7%) were found to reflect diagnoses of other conditions of anatomic proximity. Accuracy of self-reports improved with higher levels of education, but was not generally related to age, gender, race, alcohol use, or smoking. The results suggest that self-reports of some diseases can be taken as accurate, but self-reports of other conditions might require medical record verification in epidemiologic follow-up studies.

              Incidence and preventability of adverse drug events in nursing homes.

              Adverse drug events, especially those that may have been preventable, are among the most serious concerns about medication use in nursing homes. We studied the incidence and preventability of adverse drug events and potential adverse drug events in nursing homes. We performed a cohort study of all long-term care residents of 18 community-based nursing homes in Massachusetts during a 12-month observation period. Potential drug-related incidents were detected by stimulated self-report by nursing home staff and by periodic review of the records of nursing home residents by trained nurse and pharmacist investigators. Each incident was classified by 2 independent physician-reviewers, using a structured implicit review process, by whether or not it constituted an adverse drug event or potential adverse drug event (those that may have caused harm, but did not because of chance or because they were detected), by the severity of the event (significant, serious, life-threatening, or fatal), and by whether it was preventable. Examples of significant events included nonurticarial rashes, falls without associated fracture, hemorrhage not requiring transfusion or hospitalization, and oversedation; examples of serious events included urticaria, falls with fracture, hemorrhage requiring transfusion or hospitalization, and delirium. During 28,839 nursing home resident-months of observation in the 18 participating nursing homes, 546 adverse drug events (1.89 per 100 resident-months) and 188 potential adverse drug events (0.65 per 100 resident-months) were identified. Of the adverse drug events, 1 was fatal, 31 (6%) were life-threatening, 206 (38%) were serious, and 308 (56%) were significant. Overall, 51% of the adverse drug events were judged to be preventable, including 171 (72%) of the 238 fatal, life-threatening, or serious events and 105 (34%) of the 308 significant events (P < 0.001). Errors resulting in preventable adverse drug events occurred most often at the stages of ordering and monitoring; errors in transcription, dispensing, and administration were less commonly identified. Psychoactive medications (antipsychotics, antidepressants, and sedatives/hypnotics) and anticoagulants were the most common medications associated with preventable adverse drug events. Neuropsychiatric events were the most common types of preventable adverse drug events. Adverse drug events are common and often preventable in nursing homes. More serious adverse drug events are more likely to be preventable. Prevention strategies should target the ordering and monitoring stages of pharmaceutical care.
