      Effect of Video-Based versus Personalized Instruction on Errors during Elastic Tubing Exercises for Musculoskeletal Pain: A Randomized Controlled Trial

          Abstract

          Workplace interventions have shown beneficial results of resistance training for chronic pain in the neck, shoulder, and arm. However, studies have relied on experienced exercise instructors, which may not be an available resource at most workplaces. The objective of this study is to evaluate the technical performance level of upper limb rehabilitation exercises following video-based versus personalized exercise instruction. We recruited 38 laboratory technicians and office workers with neck/shoulder pain for a two-week exercise training period receiving either (1) personal and video or (2) video only instruction in four typical neck/shoulder/arm rehabilitation exercises using elastic tubing. At a 2-week follow-up, the participants' technical execution was assessed by two blinded physical therapists using a reliable error assessment tool. The error assessment was based on ordinal deviation of joint position from the ideal position of the shoulder, elbow, and wrist in a single plane by visual observation. Of the four exercises only unilateral shoulder external rotation had a higher normalized error score in the V group of 22.19 (9.30) to 12.64 (6.94) in the P group ( P = 0.002). For the remaining three exercises the normalized error score did not differ. In conclusion, when instructing simple exercises to reduce musculoskeletal pain the use of video material is a cost-effective solution that can be implemented easily in corporations with challenging work schedules not allowing for a fixed time of day to go see a personal trainer.

          Effectiveness of small daily amounts of progressive resistance training for frequent neck/shoulder pain: randomised controlled trial.

          Regular physical exercise is a cornerstone in rehabilitation programs, but adherence to comprehensive exercise remains low. This study determined the effectiveness of small daily amounts of progressive resistance training for relieving neck/shoulder pain in healthy adults with frequent symptoms; 174 women and 24 men working at least 30 h per week and with frequent neck/shoulder pain were randomly assigned to resistance training with elastic tubing for 2 or 12 minutes per day 5 times per week, or weekly information on general health (control group). Primary outcomes were changes in intensity of neck/shoulder pain (scale 0 to 10), examiner-verified tenderness of the neck/shoulder muscles (total tenderness score of 0 to 32), and isometric muscle strength at 10 weeks. Compared with the control group, neck/shoulder pain and tenderness, respectively, decreased 1.4 points (95% confidence interval -2.0 to -0.7, p<0.0001) and 4.2 points (95% confidence interval -5.7 to -2.7, p<0.0001) in the 2-minute group and 1.9 points (95% confidence interval -2.5 to -1.2, p<0.0001) and 4.4 points (95% confidence interval -5.9 to -2.9, p<0.0001) in the 12-minute group. Compared with the control group, muscle strength increased 2.0 Nm (95% confidence interval 0.5 to 3.5Nm, p=0.01) in the 2-minute group and 1.7Nm (95% confidence interval 0.2 to 3.3 Nm, p=0.02) in the 12-minute group. In conclusion, as little as 2 minutes of daily progressive resistance training for 10 weeks results in clinically relevant reductions of pain and tenderness in healthy adults with frequent neck/shoulder symptoms. www.isrctn.org/ISRCTN60264809.               When paying attention becomes counterproductive: impact of divided versus skill-focused attention on novice and experienced performance of sensorimotor skills.

              Two experiments examined the impact of attention on sensorimotor skills. In Experiment 1, experienced golfers putted under dual-task conditions designed to distract attention from putting and under skill-focused conditions that prompted attention to step-by-step putting performance. Dual-task condition putting was more accurate. In Experiment 2, right-footed novice and experienced soccer players dribbled through a slalom course under dual-task or skill-focused conditions. When using their dominant right foot, experts again performed better in the dual-task condition. However, when using their less proficient left foot, experts performed better in the skill-focused condition. Novices performed better under skill-focus regardless of foot. Whereas novices and the less-proficient performances of experts benefit from online attentional monitoring of step-by-step performance, high-level skill execution is harmed.
