      Agreement of young adults and orthodontists on dental aesthetics & influencing factors of self-perceived aesthetics

          Abstract

          Background

          The aim of this study is to assess the agreement between orthodontist’s and Chinese young adult’s self-perceived aesthetics, the normative treatment need based on the Index of Orthodontic Treatment Need (IOTN), and the main factors affecting the self-perceived aesthetics.

          Methods

          A random sample of 348 Chinese young adults (116 males and 232 females) aged 17–24 years were recruited in this study. Two orthodontists were involved in rating the cases. Orthodontic treatment need was assessed according to the Index of Orthodontic Treatment Need, including Aesthetic components and Dental Health Components (AC and DHC). Personality traits were assessed according to Eysenck Personality Questionnaire (EPQ). Cohen’s kappa test was used to assess the agreement, and spearman’s correlation coefficient was used to analyze the association among all variables.

          Results

          A statistically significant level of agreement was observed between young adult’s perception and orthodontist’s perception in IOTN (kappa = 0.14). A positive relationship ( p < 0.001)existed between the young adult’s AC and the orthodontist’ s AC ( r = 0.275), and between the young adult’ s AC and the normative need DHC ( r = 0.195). The orthodontist’s AC was strongly related to the normative need ( r = 0.743, p < 0.001). Association between the young adult’s AC and gender and EPQ-E were also observed.

          Conclusions

          Young adults tend to be less critical in assessing orthodontic treatment needs than orthodontists. The orthodontist’s AC reflecting subjective treatment need is strongly connected to the normative need. The adult’s perception of aesthetic component is affected by factors such as gender and personality traits.

          The development of an index of orthodontic treatment priority.

          The aim of this study was to develop a valid and reproducible index of orthodontic treatment priority. After reviewing the available literature, it was felt that this could be best achieved by using two separate components to record firstly the dental health and functional indications for treatment, and secondly the aesthetic impairment caused by the malocclusion. A modification of the index used by the Swedish Dental Health Board was used to record the need for orthodontic treatment on dental health and functional grounds. This index was modified by defining five grades, with precise dividing lines between each grade. An illustrated 10-point scale was used to assess independently the aesthetic treatment need of the patients. This scale was constructed using dental photographs of 12-year-olds collected during a large multi-disciplinary survey. Six non-dental judges rated these photographs on a visual analogue scale, and at equal intervals along the judged range, representative photographs were chosen. To test the index in use, two sample populations were defined; a group of patients referred for treatment, and a random sample of 11-12-year-old schoolchildren. Both samples were examined using the index and satisfactory levels of intra- and inter-examiner agreement were obtained.

            Assessment of motivation and psychological characteristics of adult orthodontic patients.

            In recent years, the demand for adult orthodontic treatment has grown rapidly; yet there is a paucity of information on this subgroup of patients. It is well known that understanding the psychological characteristics and motives of any patient is fundamental and that these factors might affect patient satisfaction and adherence with treatment. There is therefore a need for clinicians to improve their understanding of this subgroup to enhance the patient's experience of treatment delivery and to increase the potential for a successful treatment outcome. The aim of this study was to develop a measure for the assessment of motivating factors and psychological characteristics of adults seeking orthodontic treatment.

              The relationship between patient, parent and clinician perceived need and normative orthodontic treatment need.

              The aim of the present study was to compare patient, parent and clinician perceived need for orthodontic treatment in relation to normative orthodontic treatment need as measured by the Index of Orthodontic Treatment Need (IOTN). A prospective cross-sectional study was designed to address this aim. The sample comprised 103 patients attending the 'new' patient clinic at the Jordan University Hospital. The patients' mean age was 15.3 years (standard deviation 3.8 years); 33 per cent were males and 67 per cent females. One clinician scored the patients' normative orthodontic treatment need using the IOTN, then determined perceived need using a 10 cm visual analogue scale (VAS). The subjects then assessed their own perceived need and aesthetic component (AC) score and the parents carried out similar assessments for their children. All scoring was carried out blind. The parents had the highest average perceived need scores, followed by patient and clinician scores (6.6, 6.1 and 5.4 cm, respectively). A significant difference was found between the parents and the clinician (P 0.05). The present study has shown that perceptions of orthodontic treatment need are multifactorial and influenced by elements other than health measures of normative orthodontic treatment need and perceptions of aesthetics.
