          Abstract

          Se presenta y se analiza, desde el punto de vista vectorial, un electrocardiograma perteneciente a una mujer de 52 años, obesa y con hipertensión arterial sistémica. El trazo muestra un bloqueo de rama izquierda (BRIHH) de grado avanzado con ÂQRS F a + 70º. Para explicar tal comportamiento, se ha analizado la orientación espacial de los cuatro principales vectores resultantes de la activación ventricular, comparándola con la orientación habitual de dichos vectores en presencia de un BRIHH acentuado. En el caso que nos ocupa, el segundo vector, originado en la masa septal izquierda inferior, se dirige hacia abajo. Este comportamiento se debe verosímilmente a una dextrorrotación acentuada. Se ha pensado, pues, en la asociación de una probable patología del corazón derecho. Se han efectuado pruebas cardiorrespiratorias, que han demostrado la existencia de un enfisema pulmonar crónico. El estudio de este caso corrobora la utilidad de la exploración vectocardiográfica en clínica.

          Translated abstract

          An electrocardiogram corresponding to an obese, hypertensive woman 52 years old, is presented. It shows a left bundle branch block of advanced degree and the ÂQRS F at 70º. In this case, the key of the electrocardiographic diagnosis is that the second vector originated by depolarization of the lower left septal mass is oriented downward, which is unusual in the presence of left bundle branch block. This fact may be due to a marked clockwise rotation. Therefore, it is possible that right ventricle pathologic conditions be associated to those involving the left ventricle. Cardiopulmonary tests determined the existence of a chronic pulmonary emphysema. Furthermore the study of this case shows the usefulness of vectorcardiographic exploration in clinical practice.
