      Biased Sampling and Causal Estimation of Health-Related Information: Laboratory-Based Experimental Research

          Abstract

          Background

          The internet is a relevant source of health-related information. The huge amount of information available on the internet forces users to engage in an active process of information selection. Previous research conducted in the field of experimental psychology showed that information selection itself may promote the development of erroneous beliefs, even if the information collected does not.

          Objective

          The aim of this study was to assess the relationship between information searching strategy (ie, which cues are used to guide information retrieval) and causal inferences about health while controlling for the effect of additional information features.

          Methods

          We adapted a standard laboratory task that has previously been used in research on contingency learning to mimic an information searching situation. Participants (N=193) were asked to gather information to determine whether a fictitious drug caused an allergic reaction. They collected individual pieces of evidence in order to support or reject the causal relationship between the two events by inspecting individual cases in which the drug was or was not used or in which the allergic reaction appeared or not. Thus, one group (cause group, n=105) was allowed to sample information based on the potential cause, whereas a second group (effect group, n=88) was allowed to sample information based on the effect. Although participants could select which medical records they wanted to check—cases in which the medicine was used or not (in the cause group) or cases in which the effect appeared or not (in the effect group)—they all received similar evidence that indicated the absence of a causal link between the drug and the reaction. After observing 40 cases, they estimated the drug–allergic reaction causal relationship.

          Results

          Participants used different strategies for collecting information. In some cases, participants displayed a biased sampling strategy compatible with positive testing, that is, they required a high proportion of evidence in which the drug was administered (in the cause group) or in which the allergic reaction appeared (in the effect group). Biased strategies produced an overrepresentation of certain pieces of evidence at the detriment of the representation of others, which was associated with the accuracy of causal inferences. Thus, how the information was collected (sampling strategy) demonstrated a significant effect on causal inferences ( F 1,185=32.53, P<.001, η 2 p=0.15) suggesting that inferences of the causal relationship between events are related to how the information is gathered.

          Conclusions

          Mistaken beliefs about health may arise from accurate pieces of information partially because of the way in which information is collected. Patient or person autonomy in gathering health information through the internet, for instance, may contribute to the development of false beliefs from accurate pieces of information because search strategies can be biased.

          Judgment of contingency in depressed and nondepressed students: sadder but wiser?

          How are humans' subjective judgments of contingencies related to objective contingencies? Work in social psychology and human contingency learning predicts that the greater the frequency of desired outcomes, the greater people's judgments of contingency will be. Second, the learned helplessness theory of depression provides both a strong and a weak prediction concerning the linkage between subjective and objective contingencies. According to the strong prediction, depressed individuals should underestimate the degree of contingency between their responses and outcomes relative to the objective degree of contingency. According to the weak prediction, depressed individuals merely should judge that there is a smaller degree of contingency between their responses and outcomes than nondepressed individuals should. In addition, the present investigation deduced a new strong prediction from the helplessness theory: Nondepressed individuals should overestimate the degree of contingency between their responses and outcomes relative to the objective degree of contingency. In the experiments, depressed and nondepressed students were present with one of a series of problems varying in the actual degree of contingency. In each problem, subjects estimated the degree of contingency between their responses (pressing or not pressing a button) and an environmental outcome (onset of a green light). Performance on a behavioral task and estimates of the conditional probability of green light onset associated with the two response alternatives provided additional measures for assessing beliefs about contingencies. Depressed students' judgments of contingency were surprisingly accurate in all four experiments. Nondepressed students, on the other hand, overestimated the degree of contingency between their responses and outcomes when noncontingent outcomes were frequent and/or desired and underestimated the degree of contingency when contingent outcomes were undesired. Thus, predictions derived from social psychology concerning the linkage between subjective and objective contingencies were confirmed for nondepressed students but not for depressed students. Further, the predictions of helplessness theory received, at best, minimal support. The learned helplessness and self-serving motivational bias hypotheses are evaluated as explanations of the results. In addition, parallels are drawn between the present results and phenomena in cognitive psychology, social psychology, and animal learning. Finally, implications for cognitive illusions in normal people, appetitive helplessness, judgment of contingency between stimuli, and learning theory are discussed.

              Trust and Credibility in Web-Based Health Information: A Review and Agenda for Future Research

              Background Internet sources are becoming increasingly important in seeking health information, such that they may have a significant effect on health care decisions and outcomes. Hence, given the wide range of different sources of Web-based health information (WHI) from different organizations and individuals, it is important to understand how information seekers evaluate and select the sources that they use, and more specifically, how they assess their credibility and trustworthiness. Objective The aim of this study was to review empirical studies on trust and credibility in the use of WHI. The article seeks to present a profile of the research conducted on trust and credibility in WHI seeking, to identify the factors that impact judgments of trustworthiness and credibility, and to explore the role of demographic factors affecting trust formation. On this basis, it aimed to identify the gaps in current knowledge and to propose an agenda for future research. Methods A systematic literature review was conducted. Searches were conducted using a variety of combinations of the terms WHI, trust, credibility, and their variants in four multi-disciplinary and four health-oriented databases. Articles selected were published in English from 2000 onwards; this process generated 3827 unique records. After the application of the exclusion criteria, 73 were analyzed fully. Results Interest in this topic has persisted over the last 15 years, with articles being published in medicine, social science, and computer science and originating mostly from the United States and the United Kingdom. Documents in the final dataset fell into 3 categories: (1) those using trust or credibility as a dependent variable, (2) those using trust or credibility as an independent variable, and (3) studies of the demographic factors that influence the role of trust or credibility in WHI seeking. There is a consensus that website design, clear layout, interactive features, and the authority of the owner have a positive effect on trust or credibility, whereas advertising has a negative effect. With regard to content features, authority of the author, ease of use, and content have a positive effect on trust or credibility formation. Demographic factors influencing trust formation are age, gender, and perceived health status. Conclusions There is considerable scope for further research. This includes increased clarity of the interaction between the variables associated with health information seeking, increased consistency on the measurement of trust and credibility, a greater focus on specific WHI sources, and enhanced understanding of the impact of demographic variables on trust and credibility judgments.
