      INSUFICIENCIA RENAL AGUDA POR PICADURA DE HORMIGAS DEL PALO SANTO (PSEUDOMYRMEX SPP)

          Abstract

          La Insuficiencia Renal Aguda (IRA) es una complicación inusual de la picadura por hormigas secundaria a un daño tóxico isquémico con Necrosis Tubular Aguda. Los 2 pacientes presentados fueron transferidos del trópico Cochabambino al Hospital Clínico Viedma, experimentaron una reacción alérgica, hemólisis intravascular, rabdomiólisis e IRA posterior a un masivo ataque de las hormigas que habitan en el tronco del "Palo Santo" al que fueron atados por varias horas. Se dispone de pocos reportes acerca del compromiso renal en estos casos, así como de los mecanismos patogénicos implicados. Gracias al apoyo dialítico oportuno los pacientes mejoraron su condición clínica recuperando ambos la función renal en el lapso de 2 semanas de iniciada la hemodiálisis.

          Translated abstract

          Acute Renal Insufficiency (ARI) is an unusual complication caused by the puncture of ants that falls into a secondary ischemic toxic damage with an Acute Tubular Necrosis. The two patients presented were transferred from the tropic of Cochabamba state to the Viedma Hospital, they have experienced an allergic reaction, intravascular hemolysis, rhabdomyolysis and ARI caused by the massive attack of ants that inhabit the trunk of the "Palo Santo" where they were tied by several hours. Few reports are known about the affectedness of the kidney organ, as well as the implied pathogenic mechanisms. The patients improved their clinical condition thanks to the opportune support; both recovered the kidney function after 2 weeks from the initiated hemodialysis.

          Acute renal failure after massive honeybee stings

          Two clinical cases of patients who survived after numerous attacks of Africanized bees (600 and 1500 bee stings, respectively) are reported. Clinical manifestation was characterized by diffuse and widespread edema, a burning sensation in the skin, headache, weakness, dizziness, generalized paresthesia, somnolence and hypotension. Acute renal failure developed and was attributed to hypotension, intravascular hemolysis, myoglobinuria due to rhabdomyolysis and probably to direct toxic effect of the massive quantity of injected venom. They were treated with antihistaminic, corticosteroids and fluid infusion. One of them had severe acute renal failure and dialysis was required. No clinical complication was observed during hospital stay and complete renal function recovery was observed in both patients. In conclusion, acute renal failure after bee stings is probably due to pigment nephropathy associated with hypovolemia. Early recognition of this syndrome is crucial to the successful management of these patients.

            Clinical reactivity to insect stings.

            Allergy to insect stings remains a hazard worldwide and is the object of updated guidelines on management. This paper reviews the various clinical responses that may occur following an insect sting. Although the general population is at slight risk, certain groups are more susceptible, including occasionally stung adult male agricultural workers, hobby honey beekeepers and family members of beekeepers. Individuals with systemic mastocytosis are especially reactive to stings. The body of evidence attesting to the marked beneficial effect that 3-5 years of venom immunotherapy has on the natural history of hymenoptera hypersensitivity is especially evident in children. Case reports indicate other consequences of hymenoptera sting, and these are discussed. Hypersensitivity to insect stings is common and may be life threatening. Although most occur away from medical facilities, their diagnosis and management are important to a wide spectrum of health care professionals. Most reactions to stings are nonallergic manifestations of the venom's toxic effects, and present as erythema, pain and swelling about the sting site. Fire ants bite with their mandibles and pivot their head, inflicting multiple stings that usually result in a sterile pseudopustule at the site. Hypersensitivity responses to venom range from large local reactions (a late-phase response) to life-threatening anaphylaxis. Venom-specific immunotherapy is highly effective in the modification of subsequent reactions to hymenoptera stings, as is whole body extract for fire ant stings.

              Picaduras y mordeduras de animales

              En este tema tratamos las picaduras por artrópodos. Se hace referencia a las diferencias que existen entre las picaduras de avispas y abejas, comentando la composición de venenos y las diferentes reacciones locales y generales que provocan dichas picaduras. Se exponen además las picaduras-mordeduras producidas por escorpiones, arañas, garrapatas, y animales marinos con la clínica que provocan y el tratamiento que es necesario administrar. Por último, se incluyen las mordeduras por serpientes, haciendo referencia a los tipos de ofidios más frecuentes en Navarra, la forma de diferenciar la mordedura de culebras de las víboras, la diferente clínica que provocan, y el tratamiento a aplicar.
