          Abstract

          RESUMEN La atención primaria en salud es la estrategia central definida por la organización Panamericana de la Salud (OPS) para alcanzar el objetivo de "Salud Para Todos" propuesto desde 1978 por Alma Ata. En la región de las américas esta estrategia ha sido incluida dentro de los sistemas de salud y se ha interpretado y adaptado a los contextos políticos, culturales, sociales y económicos de los países en vía de desarrollo. En Colombia esta propuesta ha estado inmersa y algunas normas la definen y acotan para el desarrollo en los municipios. Bogotá ha incluido esta propuesta desde hace ya varios años, y en cada cambio de gobierno se le da una connotación particular acorde con las propuestas de los planes territoriales de salud y las propuestas en los planes de desarrollo, con mayor o menor énfasis en sus postulados. Este artículo muestra el abordaje que se da desde la atención primaria en los últimos periodos de gobierno y cómo interactúa en una pandemia como la de COVID-19.

          ABSTRACT Primary health care is the central strategy defined by the Pan American Health Organization (PAHO) to achieve the goal of "Health for All" proposed since 1978 by Alma Ata. In the region of the Americas, this strategy has been included within health systems and has been interpreted and adapted to the political, cultural, social and economic contexts of developing countries. In Colombia this setting has been immersed and some regulations define and limit it for development in the municipalities. Bogotá has included this position for several years now, and in each change of government it is given a particular connotation according to the proposals of the territorial health plans and the proposals in the development plans, with or not greater emphasis on its postulates. This article shows the approach that is given from primary care in the last periods of government and how it interacts in a pandemic such as the one presented by COVID-19.

          Community-based research partnerships: challenges and opportunities.

          The complexity of many urban health problems often makes them ill suited to traditional research approaches and interventions. The resultant frustration, together with community calls for genuine partnership in the research process, has highlighted the importance of an alternative paradigm. Community-based participatory research (CBPR) is presented as a promising collaborative approach that combines systematic inquiry, participation, and action to address urban health problems. Following a brief review of its basic tenets and historical roots, key ways in which CBPR adds value to urban health research are introduced and illustrated. Case study examples from diverse international settings are used to illustrate some of the difficult ethical challenges that may arise in the course of CBPR partnership approaches. The concepts of partnership synergy and cultural humility, together with protocols such as Green et al.'s guidelines for appraising CBPR projects, are highlighted as useful tools for urban health researchers seeking to apply this collaborative approach and to deal effectively with the difficult ethical challenges it can present.

            Urban as a Determinant of Health

            Cities are the predominant mode of living, and the growth in cities is related to the expansion of areas that have concentrated disadvantage. The foreseeable trend is for rising inequities across a wide range of social and health dimensions. Although qualitatively different, this trend exists in both the developed and developing worlds. Improving the health of people in slums will require new analytic frameworks. The social-determinants approach emphasizes the role of factors that operate at multiple levels, including global, national, municipal, and neighborhood levels, in shaping health. This approach suggests that improving living conditions in such arenas as housing, employment, education, equality, quality of living environment, social support, and health services is central to improving the health of urban populations. While social determinant and multilevel perspectives are not uniquely urban, they are transformed when viewed through the characteristics of cities such as size, density, diversity, and complexity. Ameliorating the immediate living conditions in the cities in which people live offers the greatest promise for reducing morbidity, mortality, and disparities in health and for improving quality of life and well being.
