          Abstract

          RESUMEN La Elefantiasis nostras verrucosa es una rara afectación dermatológica caracterizada por edema e hiperqueratosis maloliente con liquenificación generalizada, pápulas adoquinadas y cambios verrugosos. La patología es producida por un linfedema obstructivo crónico congénito o secundario. No existe un consenso claro para el tratamiento de esta patología, en este artículo presentamos un caso clínico de una paciente de 73 años que fue tratada con ácido tricloracético al 25% con resultados muy satisfactorios.

          SUMMARY Elephantiasis nostras verrucosa is a rare dermatological condition characterized by edema, foul-smelling hyperkeratosis with generalized lichenification, cobblestone papules, and warty changes. The pathology is produced by congenital or secondary chronic obstructive lymphedema. There is no clear consensus for the treatment of this pathology. In this article we present a clinical case of a 73 years old woman who was treated with 25% trichloracetic acid with satisfactory results.

          Lymphatic Filariasis.

          Lymphatic filariasis (LF) is a parasitic infection that is spread by mosquitos infected with worm larvae. Several factors will affect the global prevalence of LF in the future. A growing body of evidence suggests that climate change will influence the spread of parasitic diseases and their vectors. Lymphatic filariasis is the leading cause of permanent disfigurement and the second most common cause of long-term disability in the world.

            Differential diagnosis, investigation, and current treatment of lower limb lymphedema.

            The causes and management of lower limb lymphedema in the Western population are different from those in the developing world. To look at the differential diagnosis, methods of investigation, and available treatments for lower limb lymphedema in the West. A PubMed search was conducted for the years 1980-2002 with the keyword "lymphedema." English language and human subject abstracts only were analyzed, and only those articles dealing with lower limb lymphedema were further reviewed. Other articles were extracted from cross-referencing. Four hundred twenty-five review articles pertaining to lymphedema were initially examined. This review summarizes the findings of relevant articles along with our own practice regarding the management of lymphedema. The common differential diagnosis in Western patients with lower limb swelling is secondary lymphedema, venous disease, lipedema, and adverse reaction to ipsilateral limb surgery. Lymphedema can be confirmed by a lymphoscintigram, computed tomography, magnetic resonance imaging, or ultrasound. The lymphatic anatomy is demonstrated with lymphoscintigraphy, which is particularly indicated if surgical intervention is being considered. The treatment of choice for lymphedema is multidisciplinary. In the first instance, combined physical therapy should be commenced (complete decongestive therapy), with surgery reserved for a small number of cases.

              Chemical Peeling: A Useful Tool in the Office.

              Chemical peeling is a common treatment in cosmetic dermatology. A peel that has been used for many years is trichloroacetic acid. Its adverse effects have for a long time been a major limitation. We present a practical review of the characteristics, mechanisms of action, indications, and complications of superficial chemical peels and of peeling with trichloroacetic acid.
