          Abstract

          Resumen El sobrepeso y la obesidad causan gran parte de los problemas de salud en los seres humanos. Los profesionales de la enfermería ofrecen estilos de vida saludable para adecuar el peso. A través de una encuesta a profesionales de enfermería de España, se analizaron características de formación, laborales, conceptualizaciones sobre obesidad y sobrepeso, y prácticas sobre intervenciones focalizadas en este problema. Los hallazgos informan de que existe poca formación específica sobre obesidad; el uso de Guías de Práctica Clínica es muy bajo; la causa de la obesidad es atribuible mayoritariamente a factores ambientales y la edad del profesional fue la variable que mostró más asociaciones significativas con el resto de variables.

          Translated abstract

          Abstract Overweight and obesity cause many of the health problems in humans. Nursing professionals offer healthy lifestyles to adjust weight. Through a survey to nursing professionals in Spain, characteristics of training, work, conceptualizations of obesity and overweight, and practices on interventions focused on this problem were analyzed. The findings reported that there is little specific training on obesity; the use of Clinical Practice Guidelines is very low; the cause of obesity is mostly attributable to environmental factors and the age of the professional was the variable that showed the most significant associations with the rest of the variables.

          Raising the topic of weight in general practice: perspectives of GPs and primary care nurses

          Objective To explore general practitioners’ (GPs) and primary care nurses’ perceived barriers to raising the topic of weight in general practice. Design A qualitative study using the Theoretical Domains Framework (TDF). 34 semistructured interviews were conducted to explore views, opinions and experiences of initiating a discussion about weight. Content and thematic analyses were used to analyse the interview transcripts. Setting General practices located in one primary care trust in the South West of England. Participants 17 GPs and 17 nurses aged between 32 and 66 years. The modal age range for GPs was 30–39 years and for nurses, 40–49 years. Results Barriers were synthesised into three main themes: (1) limited understanding about obesity care, (2) concern about negative consequences, and (3) having time and resources to raise a sensitive topic. Most barriers were related to raising the topic in more routine settings, rather than when dealing with an associated medical condition. GPs were particularly worried about damaging their relationship with patients and emphasised the need to follow their patient's agenda. Conclusions Uncertainty about obesity, concerns about alienating patients and feeling unable to raise the topic within the constraints of a 10 min consultation, is adding to the reluctance of GPs and nurses to broach the topic of weight. Addressing these concerns through training or by providing evidence of effective interventions that are feasible to deliver within consultations may lead to greater practitioner engagement and willingness to raise the topic.

            Updating the Food-Based Dietary Guidelines for the Spanish Population: The Spanish Society of Community Nutrition (SENC) Proposal

            Diet-related risk factors and physical inactivity are among the leading risk factors for disability and are responsible for a large proportion of the burden of chronic non-communicable diseases. Food-based dietary guidelines (FBDGs) are useful tools for nutrition policies and public health strategies to promote healthier eating and physical activity. In this paper, we discuss the process followed in developing the dietary guidelines for the Spanish population by the Spanish Society of Community Nutrition (SENC) and further explain the collaboration with primary healthcare practitioners as presented in the context of the NUTRIMAD 2018 international congress of SENC. From a health in all policies approach, SENC convened a group of experts in nutrition and public health to review the evidence on diet-health, nutrient intake and food consumption in the Spanish population, as well as food preparation, determinants and impact of diet on environmental sustainability. The collaborative group drafted the document and designed the graphic icon, which was then subject to a consultation process, discussion, and qualitative evaluation. Next, a collaborative group was established to plan a dissemination strategy, involving delegates from all the primary healthcare scientific societies in Spain. A product of this collaboration was the release of an attractive, easy-to-understand publication.

              An internet-based intervention with brief nurse support to manage obesity in primary care (POWeR+): a pragmatic, parallel-group, randomised controlled trial.

              The obesity epidemic has major public health consequences. Expert dietetic and behavioural counselling with intensive follow-up is effective, but resource requirements severely restrict widespread implementation in primary care, where most patients are managed. We aimed to estimate the effectiveness and cost-effectiveness of an internet-based behavioural intervention (POWeR+) combined with brief practice nurse support in primary care.
