      Use of auxiliary devices during retreatment of direct resin composite veneers

          Abstract

          The removal of direct composite veneers, when the retreatment is necessary, represents a challenge to the clinician, since the healthy dental structure must be preserved. Thus, the aim of this study was to compare the accuracy provided by different auxiliary devices during retreatment of direct composite veneers. Seventy-five bovine teeth were prepared for direct composite veneers, scanned (T1), and restored. Specimens were divided into 5 groups for retreatment: conventional high-speed handpiece without auxiliary device (WD); high-speed handpiece with a white LED (WL); high-speed handpiece with an UV light (UL); electric motor and multiplier 1/5 handpiece (EM); and conventional high-speed handpiece using magnifying loupe (ML). After retreatments, other scanning was performed (T2). Changes on dental wear or composite residues areas, as well as, the average between wear and presence of residues were measured. Data were submitted to Kruskal-Wallis and Dunn’s post-test (p≤ 0.05). There were greater areas of wear for ML, being statistically superior to WD and EM groups. The ML presented smaller residues areas, being statistically lower than the WD and EM groups. Regarding the average between wear and the presence of resin residues, additional wear occurred after re-preparation, regardless of the group. Magnifying loupe promoted greater areas of wear and smaller areas of resin residues than conventional high-speed handpiece and electric motor. Both techniques using light accessories did not differ from other ones.

            Finish line distinctness and accuracy in 7 intraoral scanners versus conventional impression: an in vitro descriptive comparison

            Background Several studies have evaluated accuracy of intraoral scanners (IOS), but data is lacking regarding variations between IOS systems in the depiction of the critical finish line and the finish line accuracy. The aim of this study was to analyze the level of finish line distinctness (FLD), and finish line accuracy (FLA), in 7 intraoral scanners (IOS) and one conventional impression (IMPR). Furthermore, to assess parameters of resolution, tessellation, topography, and color. Methods A dental model with a crown preparation including supra and subgingival finish line was reference-scanned with an industrial scanner (ATOS), and scanned with seven IOS: 3M, CS3500 and CS3600, DWIO, Omnicam, Planscan and Trios. An IMPR was taken and poured, and the model was scanned with a laboratory scanner. The ATOS scan was cropped at finish line and best-fit aligned for 3D Compare Analysis (Geomagic). Accuracy was visualized, and descriptive analysis was performed. Results All IOS, except Planscan, had comparable overall accuracy, however, FLD and FLA varied substantially. Trios presented the highest FLD, and with CS3600, the highest FLA. 3M, and DWIO had low overall FLD and low FLA in subgingival areas, whilst Planscan had overall low FLD and FLA, as well as lower general accuracy. IMPR presented high FLD, except in subgingival areas, and high FLA. Trios had the highest resolution by factor 1.6 to 3.1 among IOS, followed by IMPR, DWIO, Omnicam, CS3500, 3M, CS3600 and Planscan. Tessellation was found to be non-uniform except in 3M and DWIO. Topographic variation was found for 3M and Trios, with deviations below +/− 25 μm for Trios. Inclusion of color enhanced the identification of the finish line in Trios, Omnicam and CS3600, but not in Planscan. Conclusions There were sizeable variations between IOS with both higher and lower FLD and FLA than IMPR. High FLD was more related to high localized finish line resolution and non-uniform tessellation, than to high overall resolution. Topography variations were low. Color improved finish line identification in some IOS. It is imperative that clinicians critically evaluate the digital impression, being aware of varying technical limitations among IOS, in particular when challenging subgingival conditions apply.

              Fluorescence properties of human teeth and dental calculus for clinical applications.

              Fluorescent emission of human teeth and dental calculus is important for the esthetic rehabilitation of teeth, diagnosis of dental caries, and detection of dental calculus. The purposes of this review were to summarize the fluorescence and phosphorescence of human teeth by ambient ultraviolet (UV) light, to investigate the clinically relevant fluorescence measurement methods in dentistry, and to review the fluorescence of teeth and dental calculus by specific wavelength light. Dentine was three times more phosphorescent than enamel. When exposed to light sources containing UV components, the fluorescence of human teeth gives them the quality of vitality, and fluorescent emission with a peak of 440 nm is observed. Esthetic restorative materials should have fluorescence properties similar to those of natural teeth. Based on the fluorescence of teeth and restorative materials as determined with a spectrophotometer, a fluorescence parameter was defined. As to the fluorescence spectra by a specific wavelength, varied wavelengths were investigated for clinical applications, and several methods for the diagnosis of dental caries and the detection of dental calculus were developed. Since fluorescent properties of dental hard tissues have been used and would be expanded in diverse fields of clinical practice, these properties should be investigated further, embracing newly developed optical techniques.
