          Abstract

          Resumen La secreción sanguinolenta a través del pezón en mujeres puede ser un signo de alarma relacionado con tumoraciones, papilomatosis o traumatismos, pero durante el último trimestre del embarazo o los primeros días del puerperio puede considerarse normal. Este cuadro clínico es conocido como calostro marrón o síndrome de las tuberías oxidadas (rusty pipe syndrome). A continuación se presenta el caso de una primigesta de 34 años que presentó telorragia bilateral tres días después de un parto eutócico a las 39 semanas.

          Translated abstract

          Abstract Bloody discharge from the nipple in women can be an alarm sign related to tumors, papillomatosis or wounds, but during the last trimester of pregnancy or in the first days of the puerperium it can be considered normal. This symptomatology is known as brown colostrum or rusty-pipe syndrome. We present the case of a 34-year-old who presented bilateral telorrhagia three days after a childbirth at 39 weeks.

          Rusty pipe syndrome, a cause of bloody nipple discharge: case report.

          Breastfeeding has universally known advantages to the mother and child that include nutritional and immunological benefits but also involve reasons of ecological and economic order. Initiation and maintenance of breastfeeding can be associated with common problems. The beginning of milk production may course with bloody nipple discharge, which, after exclusion of pathological entities, concerns a self-resolving physiological syndrome called rusty pipe. We present a case of rusty pipe syndrome and discuss the implications.

            Rusty Pipe Syndrome and Review of Literature.

            Presence of blood in colostrum may change the color of breast milk and it is known as "rusty pipe syndrome." It may resolve within days, but it may be a barrier for exclusive breastfeeding. Knowledge of "rusty pipe syndrome" among health professionals is very helpful in the management of breastfeeding initiation.

              Rusty pipe syndrome: counselling a key intervention.

              Presence of blood in the breastmilk renders a rusty or brownish colour to it; this entity is known as "rusty pipe syndrome". Although this is a self-limiting condition, it can be particularly intimidating for mothers and may act as a psychological barrier to successful breastfeeding. We describe this entity in two mothers who had spontaneous blood-stained breastmilk from both breasts in the early post-partum period and were worried about feeding their infants. Subsequent to proper counselling with the use of skills like active listening, accepting their concerns, building confidence by providing relevant information in simple language and by giving suggestions and avoiding commands, both mothers were able to successfully breastfeed their offspring.
