      Tratamiento laparoscópico en los cuadros de apendicitis aguda

          Abstract

          Se presentan los resultados obtenidos de la realización de apendicectomía por laparoscópica de 40 casos mayores de doce años de edad y de ambos sexos, que consultaron el Hospital Universitario de Caracas y al servicio de Cirugía III, por presentar cuadro de apendicitis aguda durante el período comprendido entre octubre 2002 y octubre 2004. La edad estuvo comprendida entre 16 y 50 años, siendo la edad promedio de 30.6 años. Dieciséis pacientes 40% correspondieron a pacientes del sexo femenino y veinticuatro pacientes al masculino 60%. El estado anatomopatológico más frecuente fue el flegmonoso. El tiempo operatorio promedio fue de 83.5 minutos. El promedio de hospitalización fue de 1.8 días. La tasa de complicaciones fue del 5%, correspondiendo a infección de la pared abdominal. No hubo necesidad de convertir ningún caso. No hubo mortalidad. La apendicectomía laparoscópica por ser un método seguro y efectivo, como el procedimiento abierto, se recomienda ampliamente para el tratamiento de los casos de apendicitis aguda.

          Translated abstract

          The results of the appendectomy realization are presented by laparoscoic of 40 cases bigger than twelve years old and both sexes that consulted the University Hospital of Caracas and to the service of Surgery III, to present square of appendicitis during the period understood among October 2002 and October 2004. The age was between 16 and 50 years, being the age 30.6 year-old average. Sixteen patients 40% corresponded to patient from the feminine sex and twenty-four patients to the masculine 60%. The pathological state but it frequents it was the flegmon. The time operative average was of 83.5 minutes. The average of hospitalization was of 1.8 days. The rate of complications was of 5%, corresponding to infection of the abdominal wall. There was not necessity to convert any case. There was not mortality. The laparoscoic appendectomy to be a sure and effective method, as the open procedure, is recommended thoroughly for the treatment of the cases of appendicitis.

          The First Laparoscopic Cholecystectomy

          Prof Dr Med Erich Mühe of Böblingen, Germany, performed the first laparoscopic cholecystectomy on September 12, 1985. The German Surgical Society rejected Mühe in 1986 after he reported that he had performed the first laparoscopic cholecystectomy, yet in 1992 he received their highest award, the German Surgical Society Anniversary Award. In 1990 in Atlanta, at the Society of American Gastrointestinal Surgeons (SAGES) Convention, Perissat, Berci, Cuschieri, Dubois, and Mouret were recognized by SAGES for performing early laparoscopic cholecystectomies, but Mühe was not. However, in 1999 he was recognized by SAGES for having performed the first laparoscopic cholecystectomy–sAGES invited Mühe to present the Storz Lecture. In Mühe's presentation, titled “The First Laparoscopic Cholecystectomy,” which he gave in March 1999 in San Antonio, Texas, he described the first procedure. Finally, Mühe had received the worldwide acclaim that he deserved for his pioneering work. One purpose of this article is to trace the development of the basic instruments used in laparoscopic cholecystectomy. The other purpose is to give Mühe the recognition he deserves for being the developer of the laparoscopic cholecystectomy procedure.
