      Calidad de vida en la paciente epiléptica de edad mediana Translated title: Quality of life in middle-aged epileptic patient

          Abstract

          Introducción: la epilepsia es una enfermedad incidente en mujeres en edad mediana con escasos reportes en la población cubana. Objetivos: definir factores de pronóstico de calidad de vida en mujeres epilépticas de edad mediana en consulta de climaterio y menopausia del Policlínico "Flores Betancourt", municipio Artemisa, provincia Artemisa desde enero 2011 hasta octubre 2013. Métodos: estudio casos-control seleccionando aleatoriamente 120 pacientes pareadas según la edad y divididas en 2 grupos de 60: Grupo A (con diagnóstico de epilepsia bajo tratamiento anticonvulsivante), Grupo B (no epilépticas). Calidad de vida se empleó como variable dependiente y como independientes: estado civil, ocupación, escolaridad, presencia de depresión, número de gestaciones, cantidad de hijos, severidad y duración del climaterio, el momento de la menopausia y otras patologías asociadas para una media de seguimiento de 15,5 ± 2,5 meses (rango: 0.03-18.2). Resultados: estaban deprimidas 55,8 % del total, aumentando 4,7 veces (IC 2,7-5) la depresión en las epilépticas con asociación entre variables (Mc Nemar Z= 4,87 p= 0,02). Los síntomas climatéricos intensos (OR 5,7 IC 4,7-6,6) por más de 1 año (OR 4,4 IC 4,2-5,2) fueron las entidades con mayor relación causal en el grupo A. Predominó la hipertensión arterial no controlada entre las epilépticas (OR 3,2 IC 2,6-3,1) con mayor frecuencia de osteoporosis (71,1 %). La menopausia antes de los 43 años fue la variable de mayor peso en el análisis pronóstico (Exp [B] 70,123 IC 12,456-88,987). Conclusiones: la mujer epiléptica de edad mediana enfrenta muchas dificultades físicas y psicosociales que empeoran tempranamente su calidad de vida.

          Translated abstract

          Introduction: epilepsy is incident pathology in middle age women with scarce reports in the Cuban population. Objectives: to define predictors of quality of life in epileptic middle-age women in menopause and menopause consultation at Flores Betancourt Policlinic in Artemisa from January 2011 to October 2013. Methods: a control case study was conducted selecting at random 120 patients who were pair up according to age to form two groups of 60 subjects each; Group A (diagnosed with epilepsy under anticonvulsant treatment), Group B (non-epileptic). Quality of life was used as dependent variable; and marital status, occupation, education, presence of depression, number of pregnancies, number of children, severity and duration of menopause, time of menopause were used as independent variables as well as other conditions associated to a mean follow up of 15.5 ± 2.5 months (range: 0.03-18.2). Results: 55.8 % of the total was depressed, which increased 4.7 times (IC 2.7-5) in the epileptic subjects with association among variables (Mc Nemar Z = 4.87 p=0.02). Intense climacteric symptoms (OR 5.7 IC 4.7-6.6) for more than one year (OR 4.4 IC 4.2-5.2) were the entities with more causal relationship in group A. Uncontrolled hypertension prevailed among the epileptic subjects (OR 3.2 IC 2.6-3.1) who presented higher frequency of osteoporosis (71.1%). Menopause before age 43 was the variable of more weight in the prognostic analysis (Exp [B] 70.123 IC 12.456-88.987). Menopause before age 43 was the most significant variable in prognostic analysis Conclusions: epileptic middle aged women face many physical and psychosocial difficulties early worsen their quality of life.
