          ABSTRACT

          OBJECTIVE

: To evaluate the validity and reliability of the Questionnaire for Compliance with Standard Precaution for nurses.

          METHODS

: This methodological study was conducted with 121 nurses from health care facilities in Sao Paulo’s countryside, who were represented by two high-complexity and by three average-complexity health care facilities. Internal consistency was calculated using Cronbach’s alpha and stability was calculated by the intraclass correlation coefficient, through test-retest. Convergent, discriminant, and known-groups construct validity techniques were conducted.

          RESULTS

: The questionnaire was found to be reliable (Cronbach’s alpha: 0.80; intraclass correlation coefficient: (0.97) In regards to the convergent and discriminant construct validity, strong correlation was found between compliance to standard precautions, the perception of a safe environment, and the smaller perception of obstacles to follow such precautions (r = 0.614 and r = 0.537, respectively). The nurses who were trained on the standard precautions and worked on the health care facilities of higher complexity were shown to comply more (p = 0.028 and p = 0.006, respectively).

          CONCLUSIONS

: The Brazilian version of the Questionnaire for Compliance with Standard Precaution was shown to be valid and reliable. Further investigation must be conducted with nurse samples that are more representative of the Brazilian reality. The use of the questionnaire may support the creation of educational measures considering the possible gaps that can be identified, focusing on the workers’ health and on the patients’ safety.

[Operationalizing the cross-cultural adaptation of epidemiological measurement instruments].

The objective of the article was to offer an operational framework to assess cross cultural adaptation processes of instruments developed in other linguistic, social and cultural contexts. It covers the need for using robust measurement tools; the importance of 'universal' instruments that permit cross cultural fine-tuning; and stresses the need for adapting existent instruments rather than developing new ones. Existing controversies and proposals for different procedures in current literature are reviewed and a model for adapting instruments is presented. This synthesis covers the operational steps involved in evaluating concepts, semantic and operational items, and presents psychometric analysis guidelines that underlay an evaluation of measurement equivalence. Finally, the need for adequately controlling the quality of information presented in epidemiological studies, including a meticulous cross-cultural adaptation of research agendas, is reinforced.

Factors impacting compliance with standard precautions in nursing, China

Summary Objectives To evaluate registered nurse compliance with standard precautions and to analyze the factors that affect compliance. Methods Study data were collected over a 3-month period from June to September 2007. The survey research method was used. A total of 1500 randomly sampled registered nurses from 18 hospitals in Hunan, China completed self-report questionnaires. Results Of the 1500 nurses included in the study, 1444 returned valid self-report questionnaires. Compliance with standard precautions was found to be low in the surveyed nurses. With a maximum possible score of 80, the quartile range of the overall score for compliance for all nurses was 48.29 (upper quartile score 76.36, lower quartile score 28.07), which was occupied by 64.7% of the participants. The factors most affecting compliance were: standard precautions training (odds ratio (OR) 2.17, 95% confidence interval (CI) 1.85–2.55) and knowledge (OR 1.94, 95% CI 1.01–3.41), followed by hospital grade (OR 1.61, 95% CI 1.79–1.86), presence of sharps disposal box in the department (OR 1.43, 95% CI 1.10–3.41), general self-efficacy (OR 1.29, 95% CI 1.04–1.59), exposure experience (OR 0.69, 95% CI 0.56–0.85), and department in which the nurse worked (OR 1.24, 95% CI 1.05–1.46). Conclusions The relevant authorities and hospital infection control department should pay more attention to nurse compliance with standard precautions. Standard precautions training should be strengthened and sufficient practical personal protection equipment provided in order to reduce hospital infections and protect the health of patients and medical staff.
