          Abstract

          OBJETIVOS: Analisar a condução retrógrada durante os episódios de ritmo juncional ectópico acelerado (JET) e avaliar a existência da relação entre a presença de um bloqueio retrógrado e o risco de desenvolvimento de bloqueio atrioventricular (BAV) durante os procedimentos de ablação por radiofreqüência de pacientes portadores de taquicardia atrioventricular nodal (TAVN). MÉTODOS: Foram 145 pacientes portadores de TAVN, de ambos os sexos, com 16 a 84 anos submetidos à ablação com cateter de radiofreqüência na região póstero-septal do átrio direito. Critérios avaliados: localização anatômica e comportamento eletrofisiológico da condução retrógrada durante TAVN (definindo a taquicardia como típica ou atípica) e monitorização da condução retrógrada durante o JET para eventos de risco para BAV. RESULTADOS: Dos 145 pacientes estudados, 132 (91%) preencheram critérios eletrofisiológicos e anatômicos da forma típica da TAVN e 13 (9%), da atípica. Durante a ablação, 5,3% do grupo das típicas e 30,8% das atípicas apresentaram eventos de risco para BAV. Complicações após ablação foram um episódio de BAV total e outro de BAV de primeiro grau nas típicas e um de BAV de primeiro grau nas atípicas. Os três episódios foram precedidos de eventos de risco que resultaram em uma injúria nodal após procedimento. CONCLUSÃO: Portadores de TAVN atípica apresentaram porcentualmente mais eventos preditores de risco para BAV do que os da típica (p = 0,021), sendo fundamental a observação criteriosa da condução retrógrada durante o JET, afim de se evitar permanente da condução nodal, como o BAVT, após o procedimento ablativo.

          Translated abstract

          OBJECTIVES: To analyze retrograde conduction during junctional ectopic tachycardia (JET) episodes and investigate the existence of a relationship between the presence of a retrograde block and the risk of atrioventricular block (AVB) development during radiofrequency ablation procedures in patients with nodal atrioventricular tachycardia (NAVT). METHODS: 145 male and female patients aged 16-84 years, with NAVT who had undergone radiofrequency catheter ablation in the posteroseptal region of the right atrium were studied. Evaluation criteria were anatomical location and electrophysiological behavior of retrograde conduction during NAVT, in order to understand the nodal reentrant circuit (classifying the tachycardia as typical or atypical), and monitoring of retrograde conduction during JET episodes for risk-predicting AVB events. RESULTS: Of the 145 patients studied, 132 (91%) met electrophysiological and anatomical criteria of the typical form of NAVT, and 13 (9%) of atypical form. During the ablation, 5.3% with the typical form and 30.8% of the atypical form presented risk events for AVB. After the ablation, complications were a total AVB episode in one patient and a first-degree AVB episode in another in the typical group, and one first-degree AVB in the atypical group. All three episodes were preceded by risk events and resulted in permanent nodal injury. CONCLUSION: Patients with atypical NAVT presented higher percentages of risk events for atrioventricular block than did patients with the typical form (p=0.021).A careful observation of retrograde conduction during JET episodes is vital in order to avoid permanent damage in AV nodal conduction, such as TAVB, after the ablation procedure.

          Analysis of junctional ectopy during radiofrequency ablation of the slow pathway in patients with atrioventricular nodal reentrant tachycardia.

          Junctional ectopy may occur during radiofrequency (RF) catheter ablation of the slow pathway in patients with atrioventricular nodal reentrant tachycardia (AVNRT). The purpose of the present study was to characterize this junctional ectopy quantitatively. The subjects of this study were 52 consecutive patients with AVNRT who underwent slow pathway ablation and 5 additional patients included retrospectively because they had developed high-degree atrioventricular (AV) block during the procedure. A combined anatomic and electrogram mapping approach was used for slow pathway ablation, and AVNRT was successfully eliminated in all patients. In the group of 52 consecutive patients, the incidence of junctional ectopy was significantly higher during 52 effective applications of RF energy than during 366 ineffective applications (100% versus 65%, P < .001). Compared with ineffective RF energy applications, successful RF energy applications had a significantly longer duration of individual bursts of junctional ectopy (7.1 +/- 7.1 versus 5.0 +/- 7.0 seconds [+/- SD], P < .05), a greater total number of junctional beats during the applications (24 +/- 16 versus 15 +/- 8, P < .01), and a greater total span of time during which junctional ectopy occurred (19 +/- 15 versus 11 +/- 12 seconds, P < .01). Four of the 52 patients plus an additional 5 patients developed transient AV block lasting 34 +/- 37 seconds. In 1 of the 9 patients who had transient AV block, third-degree AV nodal block requiring a permanent pacemaker recurred 2 weeks later. In each of the 9 patients who developed AV block, there was ventriculoatrial (VA) block in association with junctional ectopy during the RF energy application immediately preceding the AV block. Among 48 patients who did not develop AV block, 17 patients had at least one episode of VA block during junctional ectopy. The positive predictive value of VA block during junctional ectopy for the development of AV block was 19% in the consecutive series of 52 patients. Among 31 patients who always had 1:1 VA conduction in association with junctional ectopy, 12 had poor VA conduction in the baseline state, with a VA block cycle length of at least 500 milliseconds during ventricular pacing. In patients with AVNRT undergoing slow pathway ablation, junctional ectopy during the application of RF energy is a sensitive but nonspecific marker of successful ablation. The bursts of junctional ectopy are significantly longer at effective target sites than at ineffective sites. VA conduction should be expected during the junctional ectopy that accompanies slow pathway ablation, even when there is poor VA conduction during baseline ventricular pacing. VA block during junctional ectopy is a harbinger of AV block in patients undergoing RF ablation of the slow pathway. If energy applications are discontinued as soon as VA block occurs, the risk of AV block may be markedly reduced.

            Junctional tachycardia: a useful marker during radiofrequency ablation for atrioventricular node reentrant tachycardia.

            The aim of this study was to evaluate junctional tachycardia as a useful marker during radiofrequency ablation for atrioventricular (AV) node reentrant tachycardia. Junctional tachycardia appears to be a response of the atrioventricular node to injury and is seen during both radiofrequency AV node ablation and slow and fast pathway ablation for AV node reentrant tachycardia. We hypothesized that junctional tachycardia heralding AV node block and that associated with slow or fast pathway ablation may have different characteristics that could be useful in preventing inadvertent AV block. Characteristics of junctional tachycardia were examined after 59 radiofrequency ablation sessions in 53 consecutive patients with a mean age (+/- SD) of 41.6 +/- 16.5 years. Type 1 junctional tachycardia was followed by transient second- or third-degree AV block (n = 5) or permanent third-degree AV block (n = 1). Type 2 junctional tachycardia was followed by normal AV conduction (n = 53). Fifty-one patients had typical AV node reentrant tachycardia, and two patients had atypical tachycardia. Fast pathway ablation was attempted during 6 sessions and slow pathway ablation during 53 sessions. Patients underwent 15.3 +/- 10 radiofrequency applications, with a mean duration of 24 +/- 9.7 s. Junctional tachycardia was observed an average of 2.8 +/- 1.8 times per ablation session. Type 1 junctional tachycardia had a significantly faster rate than that of type 2 (cycle length 363 +/- 44 vs. 558 +/- 116, p < 0.001). In addition, type 1 junctional tachycardia was associated with predominantly ventriculoatrial block whereas type 2 was associated with predominantly 1:1 ventriculoatrial conduction (2 of 6 vs. 47 of 53 episodes, p < 0.05). We conclude that junctional tachycardia leading to AV block can be recognized by a faster junctional rate and ventriculoatrial block. This is a useful marker of impending AV block during slow and fast pathway ablation.
