          Abstract

          Cuidar es una actividad humana que tiene un componente no profesional; no obstante, se entiende como el corazón de la enfermería profesional. Por ello, no deja de ser asombroso comprobar cómo el cuidado se va deslizando de las manos de la enfermería para situarse, a veces exclusivamente, en manos de auxiliares y de familiares o amigos. El artículo explora la naturaleza del cuidado y el fenómeno del cuidado familiar, su contribución y lugar en el sistema de salud. Destaca que el cuidado se está convirtiendo en un asunto familiar que tiene cada vez mayor relevancia y peso. Los estudios de investigación muestran que los cuidadores familiares requieren un apoyo tanto técnico como emocional y que deben ser considerados clientes y no meros instrumentos de cuidado. El profesional de enfermería puede preparar y apoyar al cuidador para manejar las distintas etapas de la enfermedad y los cambios, puede anticipar las crisis, así como detectar prácticas de no cuidados.

          Translated abstract

          Caregiving is a human activity with a professional component. Nevertheless it is regarded as the core of nursing. Thus, it is surprising to see how care giving is slipping away from nursing hands, sometimes into the hands of nursing auxiliaries and patient’s relatives or friends. This paper explores the nature of care giving and the phenomenon of family care, its contribution and place in the health care system,stressing that family care giving is becoming a family issue with increasingly relevance and weight. Research studies have shown that family caregivers need both technical and emotional support which ought to be regarded as clients and not just means of care giving. Professional nurses could prepare and support caregivers to manage the different stages of the relative’s disease and its changes. Nurses could also anticipate crises and detect situations of poor care giving.

            Models of carers: how do social care agencies conceptualise their relationship with informal carers?

            Carers occupy an ambiguous position within the social care system. Services are predominantly structured around the dependent rather than the carer, and this has important consequences for their delivery and evaluation. Many of the problems that arise in thinking about carer issues relate to confusion over the way the relationship between social care agencies and informal carers should be perceived. The paper outlines three models that provide frames of reference for this relationship: carers as resources; carers as co-workers; and carers as co-clients. The tensions between these are then used to explore the contradictions of policy in this field.

              The health consequences of multiple roles at older ages in the UK.

              Increasing proportions of men and women are combining family (including care-giving) and work responsibilities in later life; however, the relationship between multiple role commitments and health at older ages remains unclear. Employing data from the longitudinal Retirement Survey (1988-1989 and 1994), the present authors applied logistic regression models to investigate the relationship between multiple role occupancy (1) cross-sectionally in 1988-1989 and health status in 1994; (2) retrospectively over the respondent's lifetime up to 1988-1989 and health outcomes in 1988-1989; and (3) retrospectively between 1988-1989 and 1994, and health status in 1994. The health outcomes considered were (1) general health status, (2) functional ability and (3) severity of disability category. Overall, simultaneous role occupancy (e.g. care-giving and employment) at older ages does not appear to be associated with poor health. The authors report a positive association between employment and health, as expected. There were mixed results concerning the association between care-giving and health. Where adverse health outcomes were found, the parental role, alone or in combination with other roles, was most frequently related to poor health. Thus, for a nationally representative sample of mid-life men and women, the combination of care-giving with other family and work roles appears to have few negative health consequences. Further research is needed on whether continued parental demands in mid-life have a negative impact on health.
